SUMMARY
In collaboration with patient/family, social workers, physicians and interdisciplinary team, the Social Worker II provides direct psychological, social and practical care including social work assessments, problem identification, goal setting and implementation of care plans. He / She Collaborates with all disciplines to optimize patient outcomes; scope of services may include, but is not limited to: developing action plans to mitigate psycho-social barriers to discharge; patient/family engagement and education; employing interventions (crisis intervention, grief counseling, suspected abuse reporting) to address immediate needs.

SHIFT: PRN/Casual-on call

ESSENTIAL FUNCTIONS
Reasonable accommodations may be made to enable individuals with disabilities to perform all functions. • Serves as a clinical resource expert in a specialty area; provides clinical oversight and consultation and functions as a mentor for others.
• May engage in academic work including research, grants, publications and education.
• Will be required to provide coverage during absences, vacancies and on call.
• Conducts face to face screening on all patients in assignment area with positive indicators for social work consultation.

Education : MSW required, LMSW and LCSW-C will considered

Salary Range: Minimum 42.00 per hour - Maximum 45.00 per hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.

In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
