Qualifications and description for a Social Worker, LSW / CSW:

• PA Social worker, CSW / LSW License
• Conduct Social Service assesments and documents according to state and federal regulations
• provide Physcosocial assesments and relevant parts of Minimum Data Set (MDS)
• Counseling Residents
• Gerontology / Nursing Home Experience preferred
