Social Worker - Integrated School Liaison

La Rabida Children's Hospital is hiring a full-time Social Worker - Integrated School Liaison for the Outpatient Clinic in Chicago, IL.

Job Summary:
The Social Worker/Integrated School Liaison performs various activities focused on the Stronger Together Grant, alongside the Community Health Worker/Family Advocate. They will work with La Rabida medical providers and ancillary staff as well as collaborate with CPS (Chicago Public School) professionals to assist patients throughout the IFSP (Individualized Family Service Plan)/IEP (Individualized Education Plan)/504 Plan process. They will assist in educating parents/caregivers in self-advocacy skills, and in understanding the IFSP/IEP/504 Plan process and laws. Additionally, the Social Worker-Integrated School Liaison will provide support and help in oversight of the Stronger Together Grant.

Essential Social Worker/Integrated School Liaison Functions:
  • Collaborates with primary care provider and ancillary staff (including but not limited to PT/OT/SLP and Behavioral Health) to better understand the needs of patients within the IEP/ 504 plan/ IFSP to a ssist in aligning IFSP/IEP/504 plan goals with ancillary staff (including but not limited to PT/OT/SLP and Behavioral Health) goals at La Rabida.
  • Provides family with information regarding educational rights and special education services
  • Advocates in collaboration with the parent/caregiver for patients regarding the IFSP/IEP/504 plan services and any adjustment needed for services or interventions
  • Works together with the Community Health Worker/Family Advocate to develop and provide family support groups, as indicated
  • Meets with parents/caregivers within the Outpatient Clinic, home, community, or school settings to assist with advocacy and education
  • Assists in managing and coordinating activities within the Stronger Together Grant, ensuring grant guidelines are met, working in conjunction with the grant project manager
  • Provides and documents social work services for all cases consistent with quality , productivity, and timeliness standards established by La Rabida leadership
  • Successfully participates in all mandatory training for social workers and renews licensure (if applicable) as required by La Rabida policy, grant guidelines, or state/federal regulations
  • Navigate electronic health records and other electronic programs for purposes of research, documentation, and data submission/collection/reporting.

What we need from you:

Education: Master's in social work required
Related work experience: 1 year of experience working in a school setting OR 1 year of experience working in pediatric behavioral health

Preferred work experience:
1 year of experience working as a school Social Worker OR 2-5 years of experience working in a school setting, or 2-5 years of experience working in pediatric behavioral health
Licensure/Certification: LSW or LCSW is preferred but not required. Early intervention certification is preferred but not required

Specialized Knowledge/Skills:
  • Knowledge of the IEP Process and subsequent laws, timeframes, etc. required
  • Excellent communication and interpersonal skills and demonstrated ability to work with parents/families in individual and group settings required
  • Organized and motivated self-starter being able to work independently while collaborating effectively as a team member with other healthcare professionals, and the school system required.
  • Knowledge of service delivery systems and resources for children and families, including knowledge of social services, educational services, and community services in the Chicago area preferred
  • Experience in Microsoft Office, Word/Excel required
  • Ability to communicate effectively with all required.
