Job Description

TOUR 2 (9am - 5pm)

PURPOSE OF POSITION:

To provide clinical supervision and coordination of Clinical and Administrative services for the Child Crisis Intervention Service (CCIS).

AREAS OF RESPONSIBILITIES:

Clinical Supervision and management of CCIS program at Woodhull Pediatric Emergency Room.

SUMMARY OF DUTIES AND RESPONSIBILITIES:

Under the supervision of the Director for Social Work Services or Associate Director
 Completes psychosocial database for assessment of Social Work needs using approved instruments.
 Function as child crisis specialist who is a clinical consultant in the pediatric ER, engaging the child and family and will provide initial therapeutic intervention intended to motivate both to engage in the aftercare treatment.
 Evaluates high risk /emotionally disturbed patients screened by the ED R.N. and ED physician. Provides a thorough assessment including patient's baseline knowledge of condition and treatment plan; determines patient's self-management/coping skills, identifies patient's support systems, needs and barriers.
 Assists the patient to function within the physical and emotional limitations of their illness and treatment through the maximization of formal and informal supports and coping mechanisms.
 Facilitate transition from the ER service to Outpatient care to ensure continuity of care and to reduce re-hospitalization Collaborate with team members in the planning and care of the patient.
 Educate patient and link them to appropriate agencies, ACS, School based programs and mental health clinics families about child's illness, treatment recommendations, recovery process,
 Follow up with treatment providers and patient families in regard to adherence to treatment recommendations both within the facility and in the community.
 Maintains effective communication with physicians, nursing staff, social work staff, clients/patients, families and others related to care planning.
 Provide patient with crisis counseling and advocacy, as required.
 Provides clinical supervision of the assigned social work staff of the unit.
 Insures adequate coverage and continuity of patient care at all times.
 Coordinates staff activities to meet the needs of the department.
 Participates in the formulation of standards, polices, and procedures for orientation, supervision and in-service training of social work staff.
 Provides orientation to new staff and updates incumbent staff of changes in policy, procedure, or standards.
 Knowledgeable of departmental, facility, network, city, state, federal and JCAHO standards and regulations.
 Monitors staff activities to ensure compliance with all the above.
 Provides information about and acts as linkage to available community resources.
 Participates in the department's Performance Improvement meetings and projects.
 Ensure staff documents all interventions and continuing care plans in the patient's medical record in accordance with departmental and hospital policies.
 Completes performance evaluations for assigned staff.
 Maintains and completes statistics and reports.
 Maintains a caseload as required
 Conducts and/or attends individual, unit, departmental and intra/inter hospital meetings as mandated by the department's administrator.

SKILLS:
 Thorough knowledge of social work techniques for working with individuals, groups and families.
 Ability to educate and orient staff on appropriate social work technique with a variety of populations and modalities.
 Communicates effectively with staff at all levels within the department and throughout the hospital.
 Analyzes situations, identifies problems, evaluates resolutions and implements appropriate action.
 Provides leadership and team building.
 Assists staff with development of their social work skills.
 Instructs staff on appropriate standards and methods of documentation.
 Establishes priorities and accomplish multiple tasks.
 Understands the impact of the disease process/ mental illness on psychosocial functioning

Minimum Qualifications
1. Valid license as an LCSW issued by the NYSED, and two (2) years of supervisory experience, as described above.
