Job Description

The CPEP Social Worker will provide psychiatric evaluations and social work services to patients in the Comprehensive Psychiatric Emergency Program and the Mobile Crisis Team. Conduct home visits to patients referred via NYC Well and internal resources to determine if involuntary hospitalization or community referrals are required for patient stability. Formulate and initiate appropriate discharge planning options. Responsible for all documentation that will facilitate patient's discharge. Support the service and social work department in meeting and maintaining regulatory requirements and completing surveys. Performs related duties as assigned.

Minimum Qualifications

For Assignment to Level II:
1. Valid license as an LMSW issued by the NYSED; or
2. Valid license as an LCSW issued by the NYSED.

Salary may differ based on the licensure (LMSW or LCSW) of the candidate.

Department Preferences

Experience engaging Behavioral Health patients and a desire for home and community-based treatment is a plus.
