What You'll Need

  • Master's Degree in Social Work is required.
  • Three (3) years in hospital/medical social work is preferred.
  • One (1) Year discharge planning/care management is preferred.

Licenses and Certifications
LMSW - Licensed Master Social Worker is required upon hire or
LCSW - Licensed Clinical Social Worker required upon hire
CPR - Cardiopulmonary Resuscitation is required upon hire
ACM - Accredited Case Manager is preferred
CCM - Certified Case Manager is preferred
Other ANCC is preferred

What You'll Do

You'll ensure patients are transitioned to appropriate levels of care and educate patients, caregivers, and the multidisciplinary team regarding available post-acute care services and needs. Execute and updates the discharge plan as needed.
You'll facilitate care conferences for complex transitions, placement and palliative care need and serve as a point of contact for all patient care stakeholders.

You'll participate in Joint Commission and other survey readiness activities and serves as a content expert on the following: compliance requirements for delivery of HINN, second IMM and MOON letters; Potential denials, avoidable days, and alternate level of care days; medical necessity, patient status and discharge criteria and clinical review staff requirements and communications.
