Job Description

PURPOSE OF POSITION

The Licensed Social Work Level Ill is an LCSW who as part of an interdisciplinary team on the Comprehensive Psychiatric Evaluation Program (CPEP) Crisis Outreach Program, commonly referred to as Mobile Crisis Team (MCT) which is grant funded. The mobile crisis intervention component of the CPEP offers crisis outreach and interim crisis service visits to individuals outside an emergency room setting, in the community in natural (e.g. homes), structured (e.g., residential programs), or controlled (e.g., instructional) environments. Crisis outreach service visits are emergency mental health services provided outside an emergency room which include clinical assessments and crisis intervention treatment. Interim crisis service visits are mental health services provided to individuals who are released from a CPEP for the purpose of facilitating the individual's community tenure while waiting for the first post-CPEP visit with a community-based mental health provider. The Licensed Social Worker functions as the primary evaluator of adults and children seen in the community and is also responsible for providing clinical and administrative supervision to licensed and non-licensed staff in the Social Work Department. The program operates 7 days a week between the hours of 8am - 8pm and thus the position requires the individual work Tours Il and Ill and one day of the weekend.

AREAS OF RESPONSIBILITIES

Licensed clinician with and LCSW and minimum 3 years of experience will be responsible for evaluating and assessing the severely mentally ill, chemically addicted and intellectually disabled population. Referrals are generated through inpatient units, outpatient programs, NYC Well and other referral channels within the hospital. Additionally, they will be responsible for overseeing and delegating tasks and responsibilities of direct report staff. Clinician will be responsible for completing performance evaluations of direct reports, evaluating patients in the community (shelters, homes, residences). Candidate should have a valid NYS Driver's License to operate hospital assigned vehicle and have experience working as part of a team along with knowledge of resources and services for this population. They should be able to provide crisis intervention and demonstrate good de-escalation techniques in the midst of a crisis. English proficient is a must, Spanish and Creole speaking is a plus.

SUMMARY OF DUTIES AND RESPONSIBILITIES

The Social Work Level Ill on the MCT will provide ongoing orientation to new employees, complete ongoing performance evaluations and manage time and leave for direct reports. They will conduct assessment, de-escalation, stabilization, referral to community based behavioral health and other support services for individuals who are experiencing behavioral health crisis and to avoid unnecessary interaction with police, ambulance and hospitalization as much as possible. The clinician will use a trauma informed, person centered, recovery-oriented approaches to support the individuals' capacity to manage their health & wellness, live a self-directed life, and reach their fullest potential. The duties include working in the community of Brooklyn, visiting adults and children referred for an assessment or wellness check. The clinician may be called upon at times to respond to and provide behavioral support for community disasters The clinicians goes into the community in at least a pair of 2 individuals and conducts the initial psychiatric assessment, documenting biopsychosocial assessments, formulation and disposition, facilitating supportive, psychoeducational and insight-oriented psychotherapeutic interventions, providing crisis interventions to high-risk patients, engaging with and providing support to families, and engaging them as partners in recovery. They will be required to operate a hospital assigned vehicle to use to navigate the community for patients serviced. Documenting all interventions within the required regulatory timeframes is expected. They will be responsible for Identifying and collaborating with existing support systems, facilitating community referrals as needed, collaborating with interdisciplinary treatment team to develop most appropriate case disposition, documenting all interventions within the required regulatory timeframes. They will also be required to facilitate a removal from the community for someone experiencing a psychiatric crisis. When needed, they may also be assigned to provide coverage to the CPEP by completing evaluations and re-evaluations. Good writing skills and ability to work in a fast-paced environment required with effective time management skills is necessary. English proficient a must; ability to communicate in Spanish or Creole a plus.
