Per Diem

Summary:
Assists patients and families with their adaptation to illness, hospitalization and the medical regime. Ministers to the spiritual needs of patients/families in conjunction with pastoral services. Facilitates the transition from hospital-to-home or for other levels of care by utilizing an array of support services.

Minimum Qualifications:

An individual that is a graduate of an accredited School of Social Work, currently licensed or pursuing license in the state of Connecticut. Minimum of 2 years experience in acute care, long-term care or home care is required.
