Department-Specific Responsibilities
- Provides medical social work services to participants and their families including psychosocial assessment, solution-focused interventions, crisis intervention services, trauma-focused care, discharge planning, information and referral, participation in multidisciplinary treatment team, and coordination of community resources.
- Participates in consultative multidisciplinary treatment team, coordination of community resources, maintains database of participant population, intervention, and outcomes.
- Coordinates follow-up care and consultation with allied health professionals and community partners.
- Documents participant activity as required by Healthy Beginnings at Home protocols.
- Serves as liaison to community agencies regarding participant barriers or issues related to housing and healthcare.
- Identifies and pursues opportunities to enhance knowledge base related to programs, articles, staff education, or community projects.
- Complies with all policies and standards, accurately documenting and serving as a resource when needed;
- Complies with required standards for medical/legal reporting situations, and/or educates other professionals with responsibility for reporting.
- Conducts individual intake assessments
- Provides direct clinical mental health services to individuals, groups, and couples
- Manages individual caseload so as to be able to incorporate new clients in an ongoing manner
- Maintains accurate, timely clinical notes, following Quality Improvement committee standards
- Adheres to relevant ethical and legal standards of HIPAA, relevant professional bodies, state boards
- Consults with and refers to other professionals when appropriate (e.g. psychiatrists, medical providers, and dietitians)
- Utilizes evidence-based clinical interventions and, as appropriate, document in clinical notes
- Serves on emergency response teams
- Participates in active educational outreach programs
- May supervise Master's level mental health students
EDUCATION
Required
- Master's degree in relevant mental heath area from an accredited program

- 2 years of post-degree experience
Required
- Licensed Clinical Social Worker in the state of Indiana or license eligible upon date of hire
- Licensed Mental Health Counselor in the state of Indiana or license eligible upon date of hire
