Job Description

SCOPE OF POSITION
The Social Worker, Wexford House will assist in planning, developing, organizing, implementing, evaluating, and directing the social service programs of this facility. This individual will participate in community planning related to the interests of the facility and the services and needs of the resident and family. The Social Worker will participate in discharge planning, development, and implementation of social care plans and resident assessments. The Social Worker will interview residents/families to obtain social history. Additionally, the Social Worker will provide consultation to members of our staff, community agencies, etc., in efforts to solve the needs and problems of the resident through the development of social service programs. The Social Worker will perform other duties as assigned within the scope of practice.

EDUCATION AND EXPERIENCE
Bachelor's degree in Social Work or a Human Service field including but not limited to Sociology, Special Education, Rehabilitation, Counseling, and Psychology.
