Schedule: Part-Time, 20 hours per week

Shift : Primarily Mon-Fri, Two to Three days a week, with some weekends.

The value you'll bring to the team:

The Home Health ST Care Manager is a professional nurse who coordinates and directs the home care patient's care based on individual patient needs. Care Manager is responsible for independent management of the Home Health patient population requiring the use of advanced assessment, teaching and decision-making skills. The nurse is responsible for ensuring that appropriate referrals to other services are made, interdisciplinary conferencing takes place regularly, and appropriate documentation is completed. Relevant knowledge and experience is consistently applied to new patient populations. The Care Manager cares for a caseload of home health patients by evaluating the patient for appropriateness of home health and developing the home care plan in conjunction with the physician. S/he educates patients, families, caregivers and community providers to safely perform care. S/he provides follow up by evaluating effectiveness of the home care plan, and monitoring patient/family's response to the plan to achieve patient/family goals and top decile outcomes. The Care Manager also identifies performance improvement and home health standard of care initiatives and assists to design or implement programs to address needed changes.

The role you'll contribute:
  • Performs an evaluation and utilizes the findings to determine diagnosis (for SLP services), prognosis and plan of care in consultation with the physician. Determines specific interventions, duration and frequency needed to achieve anticipated goals and outcomes.
  • Observes and records activities and findings in the clinical record and reports to the physician and or primary Care Manager the patient's reaction to treatment and any changes in patient's condition, or when there are deviations from the plan of care.
  • Instructs the patient and caregiver regarding the patient's speech language improvement program including adaptive equipment and the home program.
  • Instructs other health team personnel including, when appropriate, home health aides and caregivers in certain phases of speech therapy with which they may work with the patient.
  • Selects, applies or modifies skilled intervention to the patient using various skilled interventions consistent with the state practice act to achieve the best outcome possible for the patient. Assesses patient response to interventions and performs reassessments as required.


Qualifications
The expertise and experiences you'll need to succeed:

Minimum qualifications :

• Functions with a high degree of independence

• Ability to delegate tasks to appropriate personnel as indicated by skill level and professional standing

• Strong computer and technology skills

• Minimum of one-year relevant clinical Speech therapy experience

• Master's degree in Speech Therapy

• Current Speech Therapy License in State of Practice

• Valid Driver's License and current car insurance

• CPR certified

SUPERVISORY RESPONSIBILITIES
  • Field supervision: Effectively delegates and supervises delegated care, e.g., care provided by the home health aide and speech therapy assistants.


Preferred qualifications:

• A working knowledge of community resources and an ability to refers patients and families appropriately

• Home Care Regulations and Third-Party Reimbursement as it impacts care delivery

• Home Health Case-Manager Certification

• COS-C

• Recent, relevant experience in a Medicare-certified home health agency as a case-manager
