Responsabilities:

• Assessment and Evaluation: Conduct thorough assessments of residents' speech, language, cognitive, and swallowing abilities.

• Individualized Care Plans: Develop customized intervention plans to address communication and swallowing difficulties.

• Section GG Data Collection: Ensure accurate and consistent collection of Section GG data for Annual, Quarterly, Falls, and Significant Change assessments, Section K, BIMS, PhQ2-9, and other clinical data as applicable.

• Treatment: Administer speech therapy interventions, including exercises, communication strategies, and swallowing therapy.

• Documentation: Maintain accurate and timely documentation of resident progress and any interventions provided. Comfortable and knowledgeable with Point of Service( PoS) care delivery and documentation. Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, HMO/Manage care progress updates in EMRs( Net Health/PCC).

• Education: Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conducts remote check-in through remote therapeutic monitoring(RTM) with discharged patients to home or next level of care.

• Interdisciplinary Collaboration: Collaborate with other rehab staff, such as Physical Therapists and Occupational Therapists, to provide holistic care for residents.
