Job Description

PURPOSE OF THE POSITION:

The Staff Nurse is a Nursing Practitioner who contributes to the development, implementation and evaluation of the nursing regimen for assigned patients on his/her unit. Under the direction and leadership of the Head Nurse the Staff Nurse is guided by a thorough knowledge of nursing theory, principles and standards of quality nursing care.

RESPONSIBILITIES:

(1) Provide care to assigned patients through assessment of patient needs, developing and implementing an interdisciplinary plan of care, which reflects an individualized mode of nursing and medical intervention so that the appropriate outcome can be achieved in a timely manner.

(2) Initiates a Nursing Assessment and interdisciplinary treatment
discharge plan upon admission, which reflect patient teaching and discharge plan according to established guidelines.

(3) Allows patient and/or significant others, as much as possible to collaborate in plan of care. Develop discharge plan in collaboration with physician, patients/family other
discipline utilizing available resource.

(4) Execute physician orders concerning medication and therapy and evaluates patient response with a clear understanding of the scientific principles of cause and effects.

(5) Collaborates with the Head Nurse in delegating assignments for nursing personnel based on patients' acuity and employee skill level.

(6) Records pertinent observations on patient chart and reports findings to Head Nurse and/or physician.

(7) Evaluates the results of nursing intervention and males appropriate changes when necessary.

(8) Able to monitor suicidal patients and document observations as per hospital policy,

(9) Administer medications and treatments in safe knowledgeable manner and document same according to hospital policy, procedure and professional standards.

(10) Prepare for and assist with diagnostic procedures and treatment.

(11) Demonstrates proficiency in insertion of peripheral cannulation, and administration of nebulizer and/or metered dose inhalation therapy.

(12) Count narcotic with on-coming and out-going tour nurses according to hospital
pol icy/procedure.

(13) Check code cart/emergency equipment on a daily/tour by tour basis-open according to service protocol.

(14) Counsel and/or instruct patients and/or family with evidence of the instructions and the patients or families understanding of the instructions documented in the progress note and teaching tool.

(15) Evaluates interdisciplinary treatment discharge plan on an ongoing basis and makes modification as indicated.

(16) Conforms to policy governing patient abuse, patient rights, and confidentiality of information.

(17) Assures that the instructions and counseling given to patient are consistent with that of the responsible Medical Practitioner.

(18) Participates in in-service department and continuing education program in order to foster procedures affecting nursing practice.

(20) Participates in performance assessment and improvement activities on an ongoing basis and assist with data collection.

(21) Maintains professional competence through participation in continuing education, in-service, staff development and self-directiveness in his/her own education process.

(22) Assist in coordinating and discharge planning monitoring activity for clinical services. Assists in providing ongoing technical assistance to interdisciplinary patient care team. Assist in planning, directing and coordinating patient care; assume responsibility for the care, comfort and cleanliness of patients.

(24) Able to assume responsibility for the acutely/critically ill patients.

(25) Assessment and completion of weekly patient classification and pressure ulcer report.

(26) Able to assume responsibility for assisting in the supervision of non-professional assistants and orientation of new staff

(27) Function as nurse in charge of the unit as districts nursing/modified primary nursing.

(28) Accompany patients to MRI on Ambulette/ Ambulance when necessary.

(29) Participates in Clinical Pathways, initiating and completing nursing flow sheet, e.g., Fall Precaution Flow Sheet, Restraint Flow Sheet, I&O Sheet, Graphic Sheet, Intermediate Flow sheet if applicable.

(30) Conforms to policies governing uniform codes, attendance and punctuality.

(31) Demonstrate sensitivity to diversity in work force,

(32) Ensures a safe and secure environment free of hazards to reduce the risk of injury to all customers. (Patients, visitors and staff).

(33) Conducts risk assessments to reduce ad verse events.

(34) Ensures that safety management issues are appropriately addressed.

(35) Assess and monitors patient on telemetry. Obtains rhythm strip. Notifies MD of changes.

(36) Provides care of patients but not limited to Respirator, Cardiac Neurological and Gl Renal HIV.

(37) Start peripheral venous access maintain intravenous therapy and site care.

Minimum Qualifications
1. A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and
2. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
