Job Description

PURPOSE OF POSITION:

The staff nurse in Behavioral health is a Registered Nurse who contributes to the development, implementation and evaluation of nursing care for the assigned patients in his/her unit. Under the direction and leadership of the Associate/Assistant Director of Nursing or the Head nurse, the staff nurse is guided by a thorough knowledge of nursing theory, principles and standards of quality care.

RESPONSIBILITIES:

  • Accountable for the care, safety and goal achievements of assigned patients through assessing, diagnosing, planning and interviewing in actual or potential health problems and evaluation patient's response to care.
  • Conducts an individualized patient assessment based on the patient's immediate needs and within the facility's timeframe policy.
  • Conducts ongoing assessments as determined by the patient's condition and reprioritizes care accordingly. GOAL: completes care activities on time, demonstrates organization in completing all care/tasks.
  • Develops a plan of care that is individualized for the patient reflecting collaboration with other members of the healthcare team.
  • Documents patient assessment findings, behavioral/ psychosocial response to nursing intervention and progress towards problem resolution. Maintains accurate and detailed reports and records.
  • Always checks patient identification prior to providing care or service. Assures patient has identification band.
  • Accepts in person reports from departing nurse on patient progress, needs, medications and plan of care.
  • Endorses patient care status and need to RN assuming responsibility during absence at lunch, break or meeting.
  • Reviews individual patient/family plan of care in conjunction with information from the departing nurse and establishes priorities.
  • Reports pertinent and concise patient condition to appropriate personnel during each shift and reports any changes in timely manner.


Minimum Qualifications
1. A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and
2. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
Special Note:
For assignments in Post-Acute Care ONLY, individuals meeting the following criteria may also be considered for appointment: Graduation from a nursing education program registered by the New York State Education Department (NYSED) as licensure qualifying for Registered Professional Nurse (RN) and has applied for licensure as an RN. These unlicensed individuals may be employed to practice under the immediate and personal supervision of a NYS licensed, currently registered, RN for up to 90 days immediately following graduation pending receipt of a limited permit, which selected candidates must apply for and receive within the same 90-day period. Once a limited permit is received, individuals can practice nursing, under the immediate and personal supervision of a NYS licensed, currently registered, RN at a NYC Health + Hospitals health care setting for up to one (1) year from the date of issuance of the limited permit. In addition, must hold or obtain through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
