NYC Health + Hospitals/Gouverneur provides primary and specialty outpatient care through its Diagnostic and Treatment Center (D&TC) and residential care for the elderly and infirmed in its 295-bed Skilled Nursing Facility (SNF). Gouverneur recently completed a $250 million modernization project to offer enhanced, cutting edge healthcare services in a state of the art space. NYC Health + Hospitals/Gotham, Gouverneur's D&TC is the largest freestanding D&TC in New York, providing more than 250,000 visits per year on the Lower East Side. Gotham Health| Gouverneur provides a comprehensive array of on-site services, including primary care, pediatrics, women's health, dentistry, behavioral health, podiatry, optometry, ENT, digital imaging, infectious disease, and other specialties and sub-specialties. NYC Health + Hospitals/Gouverneur Skilled Nursing Facility provides 24-hour care for individuals in need of short-term, long-term care or hospice care. The facility is rated as a Best Nursing Home by US News and World Report. Post-Acute Care services include physical therapy, occupational therapy, speech and audiology, and vocational rehabilitation.

Job Description

Under supervision, provides professional nursing care to patients/clients within an assigned unit. Diagnoses and treats responses to actual or potential health problems by means of nursing methods and techniques such as case-finding, health teaching, health counseling and the provision of care supportive to and restorative of life and well-being.
1. Plans, implement and evaluates nursing care utilizing the nursing process.
2. Formulates and implements comprehensive and nursing care plan. Sets priorities, delivers and directs care to meet resident needs.
3. Evaluates residents' responses and makes appropriate changes on the care plan, on an on-going basis. Maintains accurate documentation as described in policies and procedures.
4. Collaborates with other members of interdisciplinary team in the care of residents, including the resident, family, and/or significant other's. Discuss education and discharge planning issues as they arise.
5. Transcribes physician's order and administers medication/treatment as defined in all policies.
6. Assume charge nurse responsibilities as assigned.
7. Maintains professional competence through participation in continuing education and other appropriate learning experiences. Attend all mandatory classes and participate in the orientation/training staff.
8. Demonstrates safety awareness and follows protocols for fire, disaster, infection control and other safety practices.
9. Participate in performance improvement activities and other committees as assigned.
10. Performs other related duties.
11. Supervises and directs Licensed Practical Nurse, Medical Surgical Technician, Certified Nurse's Aide or other allied nursing personnel concerning the care of residents.

Minimum Qualifications

For individuals holding an unrestricted registered professional nursing license BEFORE July 1, 2020 in New York State, another state or United States territory:
1. A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and
2. Valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).

For individuals issued a registered professional nursing license on or AFTER July 1, 2020, based on expected implementation of Chapter 502 of the Laws of 2017 and Chapter 380 of the Laws of 2018:
1. A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and

a. A Bachelor's of Science degree in Nursing or higher (or other nursing degree, as identified by NYS Education Department including Master of Science in Nursing, Doctor of Nursing Practice, Doctor of Philosophy Degree in Nursing or Doctor of Nursing Science) from an accredited college or university, registered with the NYSED; or

b. Obtains a Bachelor's of Science Degree in Nursing or higher, as described in "a" above, within ten (10) years of the date of initial licensure; and
2. Valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).

Additional Qualifications

For assignments in Post-Acute Care ONLY, individuals meeting the following criteria may also be considered for appointment: Graduation from a nursing education program registered by the New York State Education Department (NYSED) as licensure qualifying for Registered Professional Nurse (RN) and has applied for licensure as an RN. These unlicensed individuals may be employed to practice under the immediate and personal supervision of a NYS licensed, currently registered, RN for up to 90 days immediately following graduation pending receipt of a limited permit, which selected candidates must apply for and receive within the same 90-day period. Once a limited permit is received, individuals can practice nursing, under the immediate and personal supervision of a NYS licensed, currently registered, RN at a NYC Health + Hospitals health care setting for up to one (1) year from the date of issuance of the limited permit. In addition, must hold or obtain through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA). Staff Nurses holding a limited permit and Staff Nurses awaiting their limited permit to practice nursing, will be paid $60 annually below the rate paid of a NYS licensed Staff Nurse pending receipt of a NYS license to practice as a Registered Professional Nurse.
