Job Description
1. Ensures dress code meets department policy.
2. Always reviews, understands and follows instructions in the residence plan of care before performing care/assistance to a resident.
3. Always documents in the electronic medical record (POC), in an accurate manner, any care/assistance provided to a resident promptly after providing the care/assistance.
4. Ensures residents are protected from any abuse, neglect, mistreatment, misappropriation of resident property or exploitation and reports immediately any of these issues to their supervisor immediately.
5. Directs, plans and supervises the nursing regimen of resident care in the absence of the Head Nurse. Plans and provides nursing care to a number of residents assigned, in accordance with the nursing care plan. Assists the provider with treatment, dressings and physical examinations. Administers medications and treatments as indicated by the provider's orders and work assignment.
6. Maintains accurate records by charting medications, treatment, and observations made of resident's condition. Notifies the charge person of any serious change in resident's condition and, if necessary, takes the required remedial action in emergency situations: formulates and implements the Nursing Care Plan and notifies appropriate members of IDC team.
7. Evaluates continuously the effectiveness of nursing care being given assigned residents.
8. Makes provision for an optimum physical environment for his/her residents in conformance with facility policy.
9. Assists with the direction of ancillary workers assigned to the unit. Provides guidance and assistance to them in their respective areas of resident care. Shares in the total cleanliness and tidiness of unit as a whole. Reviews completion of their assignments at end of tour, Nurses Aide Accountability record (NAAP, I&O, Toileting, etc.).
10. Records all administered controlled drugs to insure all medications are accounted for promptly. Notifies MD if re-orders are needed.
11. Sets an example of acceptable nursing service standards by the care given to residents and by personal conduct.
12. Consults with nurse in charge in instances of repeated poor and/or excellent employee performance.
13. Assists in carrying out studies on the unit and participates in Performance Improvements Programs.
14. Seeks help of the Head Nurse, Assistant Head Nurse, or ADN as indicated in problems related to nursing care.
15. Keeps informed of facility policies and supports them. Interprets these policies to visitors, residents and co-workers.
16. Participates in Team Conferences and plans for individualized resident care. Shares with co-workers knowledge of resident's condition(s), reaction(s), nursing care rendered and plans for future care. (Changes in skin condition, weight loss behavior problems). During change of shift report/24-hour report.
17. Attends In-Service Education Programs, meeting and other conferences as assigned, in order to increase knowledge.
18. Writes Nursing Care Plan/Monthly Summary as assigned using guidelines for documenting and may be required to write goals and plans for residents in the comprehensive care plan.
19. Performs other related duties as assigned by the ADN and be required to carry out emergency assignments when necessary for adequate staffing coverage.
20. Participates in restorative nursing techniques, eg. ambulation, transferring residents to wheelchair, range of motion, activities of daily living, toileting programs.
21. Assures that the quality of Nursing Care administered on the unit is in accordance with Nursing Service Standards.

22 . Seeks guidance from the Assistant Director of Nursing (ADN) in absence of Head Nurse.
23. Completes assigned MDS + forms quarterly, annually and PRN.
24. Reviews residents scheduled for clinic appointments and assigns staff to accompany them.
25. Reviews Medication Administration Records and treatment records for completeness and accuracy at the end of tour.
26. Checks emergency equipment daily (suction machine, EKG machine, etc.)
27. Checks that there is an adequate amount of supplies for patient care and orders what is needed.
28. Supervises unit clerk in performance of responsibilities (clinic appts, name tags for residents, organization of files, etc.)
29. Performs other duties within nursing practice guidelines, as directed by Head Nurse.
30. Follows Infection Control Policies and Procedures.

Minimum Qualifications
1. A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and
2. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
Special Note:
For assignments in Post-Acute Care ONLY, individuals meeting the following criteria may also be considered for appointment: Graduation from a nursing education program registered by the New York State Education Department (NYSED) as licensure qualifying for Registered Professional Nurse (RN) and has applied for licensure as an RN. These unlicensed individuals may be employed to practice under the immediate and personal supervision of a NYS licensed, currently registered, RN for up to 90 days immediately following graduation pending receipt of a limited permit, which selected candidates must apply for and receive within the same 90-day period. Once a limited permit is received, individuals can practice nursing, under the immediate and personal supervision of a NYS licensed, currently registered, RN at a NYC Health + Hospitals health care setting for up to one (1) year from the date of issuance of the limited permit. In addition, must hold or obtain through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
