Job Summary

Develop, implement and sustain the disease management program for the adult stroke disease management program across the continuum of care. In collaboration with the interdisciplinary team, develops, maintains and monitors stroke practice, guidelines, and order sets. Design and analysis of utilization opportunities, development of strategies to maintain compliance with Primary Stroke Center Certification, participates in the design of the internal/external marketing of the program, and provides education for nurses involved in the care of stroke patients.

Responsibilities

• Facilitate the timely implementation of the plan of care in collaboration with the multidisciplinary team. • Educate multidisciplinary team regarding documentation in order to ensure continuity of care and evidenced based practice. • Interface with attending and consulting physician, residents, staff nurses, and other members of the multidisciplinary team to address system processes which impact the timely facilitation and delivery of care. • Works in direct collaboration with the Stroke Medical Director, Nurse Manger of Neurological Services and hospital administration for the Stroke Center of Excellence in support of day-to-day operations, clinical management program planning, development and marketing. • Provide in-service education and serve as a resource for physicians, residents, medical students, nursing, and other professional staff. • Participate in performance improvement activities in order to share data and develop methods to optimize resource utilization in order to achieve quality outcomes. • Utilize tracking methods to facilitate system changes or change in practice and participate in quality management activities, including screening, focus studies and others. • In collaboration with the interdisciplinary team, develops, maintains and monitors stroke practice, guidelines, and order sets. • Perform work in a cost-effective manner. • Incorporates data and information to continuously improve care and practice to enhance outcomes. • Identifies and facilitates the provision of stroke education and informational stroke programs for nursing, allied health personnel, and families. • Annually, work with marketing to review and revise marketing strategies for the Stroke Center. Participates in the design of the internal/external marketing of the program. • Communicate with administration, Stroke Medical Director, physician and Nurse Manager of acute care unit regarding specific patient care issues. • Provide community outreach activities for education and health screenings of participants.

• Associate's Degree (Required) Experience: 5-7 Years Nursing Experience
