Position Highlights

• Work location: Texas Health Azle at 108 Denver Trail Azle TX 76020
• Work hours: Full-time, 40 hours weekly, Monday-Friday, 8:00 AM to 5:00 PM

What You'll Need

• Bachelor's degree in Nursing if an RN is required. (Individuals hired in Supv Care Transition position prior to May 11, 2017, will be grandfathered with an ADN at the entity they were employed at on May 11, 2017.)
• Or a Master's Degree in Social Work if an LMSW is required.
• Three (3) years of staff nurse experience at an acute care hospital (if RN) is required.
• Three (3) years of social work experience in an acute care hospital/medical setting (if social worker) is required.
• Two (2) years hospital discharge planning/care management experience is required.
• Case management supervisory background preferred.
• RN - Registered Nurse upon hire required for an RN.
• LMSW- Licensed Master Social Worker upon hire required for a Social Worker.
• CPR - Cardiopulmonary Resuscitation upon hire required.
• ACM - Accredited Case Manager upon hire preferred.
• CCM - Certified Case Manager upon hire preferred.
• ANCC upon hire preferred.

What You'll Do

• Texas Health Azle Care Transition Supervisors are responsible for ensuring patients are transferred to appropriate levels of care in a timely and effective manner.
• Communicate with the multidisciplinary team (physicians, nursing, therapy), patient, family and post-acute care stakeholders in order to coordinate care.
• Facilitate care conferences for complex transitions, placement and palliative care needs.
• Serve as a point of contact for all identified stakeholders.

• Assists leadership to remedy actual vs. expected customer satisfaction performance gaps as needed
