Job Summary:

The Training Specialist for Lifestyle Change Programs is responsible for exemplifying the University's Mission, Vision, and Values, providing outstanding customer service. The position will function as an integral part of the interprofessional team working closely with Lifestyle Change Program leadership and staff. The Training Specialist will function as a member of the team that trains and assists with conducting Non-Medical Drivers of Health (NMDoH) assessments while providing guidance and support to CHWs and other staff in completing NMDoH assessments on patients as well as data entry for NMDoH assessments. The position assists with the blood pressure lending library clinical and community programs.

Primary responsibilities of the position include community outreach and awareness, recruitment and follow-up, facilitation of health improvement, health promotion, health program development and support, prevention activities, and collaboration with regional and local partners within the community. This position works as an interdisciplinary team member and reports directly to the Program Manager.

This position requires some evening, weekend, and travel to various clinical locations within the six (6) census tracks as well as for outreach, recruitment, and health promotion.

Major Responsibilities / Duties / Critical Tasks:

  • Provide guidance and support to other CHWs and staff in the clinics in completion of Non-Medical Drivers of Health Assessments.
  • Complete documentation and required reporting for all services to include the NMDoH assessments in the specific database and/or Excel spreadsheet within given timeframe.
  • Participate in community outreach and deliver/assist with screenings in the community.
  • Promote health education and encourage healthy lifestyles for preventing and managing cardiovascular disease.
  • Train and assist with conducting home visits, when necessary, i.e., follow-up, health education, application assistance, etc.
  • Train and assist with assessment of individuals/patients for potential eligibility for grants, programs, and/or community resources.
  • Train and assist with completion of forms/applications for individual/patients required to receive needed services.
  • Train and assist with scheduling individual/patients' future appointments and linking to providers and/or needed services.
  • Train and assist with providing individual/patients referrals to external community resources (such as food pantries) as needed.
  • Train and assist with providing follow-up for individuals/patients within the given timeframe.
  • Speak with/present community groups/organizations and diverse populations about the program in a sensitive and culturally competent manner.
  • Assist with development, improvement, and standardization of processes and guidelines.
  • Collaborate with other team members and stakeholders to promote population health.
  • Handle all phone calls, emails, and in-person interactions cordially and professionally.
  • Remain current on chronic disease and behavioral health trends, business practices, and health education and promotion theory.
  • Communicate effectively with team, the Center, and UTT HSC colleagues, project partners, stakeholders, subcontractors, and consultants to achieve outcomes, metrics, and improvement targets.
  • Coordinate with director supervisor, project coordinator, and/or assigned leadership educational and outreach events focusing on project goals and requirements.
  • Handle administrative functions to promote, coordinate, collaborate, and support the scope of work required to achieve stated goals.
  • Use excellent oral and written communication skills to prepare memos, formal correspondence, acknowledgements, notes, and reports to interact with team members efficiently and effectively, internal, and external partners to accomplish essential tasks.
  • Maintain professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; benchmarking state-of-the art practices; participating in professional societies as deemed appropriate.
  • Handle confidential/sensitive information with discretion and adherence to HIPAA rules.
  • Teach group and/or one-on-one self-management of cardiovascular disease with the six (6) designated census tracks.
  • Participate in team meetings as scheduled.
  • Performs other duties as assigned.

Qualifications

Required Education / Experience:
• High School diploma or GED required.
• Bachelor's degree preferred.
• Preferredcurrent Community Health Worker certification by the Texas Department of State Health Services required. Preferred maintenance of current CHW certification.
• Two (2) years of healthcare and/or clinic experience.
• Two (2) years of experience conducting and promoting health education preferred.
• Basic knowledge of standard office equipment.
• Basic knowledge of resources available for individuals/patients (state/federal/local assistance programs).

Required license/certification:
• Preferred current Community Health Worker certification by the Texas Department of State Health Services required. Preferred maintenance of current CHW certification.
