Job Description Summary
The Department of Surgery is actively recruiting for a part-time Early Violence Prevention & Intervention Referral Coordinator. This individual will be responsible for coordinating high-risk pediatric referrals to MUSC’s Turning the Tide Violence Intervention Program (TTVIP), a hospital-based violence intervention program (HVIP), for early intervention. Based out of MUSC Health - Charleston’s Level 1 adult and pediatric trauma centers, TTVIP aims to prevent and reduce community and interpersonal violence among youth and young adults by providing comprehensive wrap-around, multidisciplinary services to address patients’ underlying risk factors and social determinants of health. The purpose of this role is to field incoming referrals, assess patients’ eligibility for TTVIP long-term intervention enrollment, provide support and resources to patients and families, and coordinate enrollment with TTVIP’s early intervention client advocate(s). This is a part-time (20 hours/week), benefits eligible role with hybrid work opportunities.

$31,561.00 - $44,977.00 - $58,393.00 (min - mid - max)

Job Duties:
  • 35% - The role will collaborate with the clinical care team to complete bedside introductions of TTVIP and perform eligibility assessments, in addition to calling patients/families for follow-up from clinic-based referrals. For patients that don’t meet eligibility criteria, this role will be responsible for providing appropriate referral resources for any needs or risk factors identified during the assessment.
  • 25% - The role will work directly with patients and families who are identified as having multiple risk factors for violence exposure in MUSC Health - Charleston’s pediatric inpatient and outpatient settings. This role will include coordinating referrals received from MUSC providers, which will include screening patients for risk of violence, maintaining referral queues, coordinating clinic visits, and ensuring loop closure with referring providers.
  • 20% - The referral coordinator will collaborate closely with the early intervention client advocate to facilitate warm hand-offs of eligible patients whenever possible. Warm hand-offs may include this role facilitating bedside introductions, or scheduling phone appointments or home visits, between the patient/family and client advocate.
  • 15% - This role will document all referral coordination activities in patients’ medical charts and TTVIP’s case management databases. This data is recorded for the purposes of documentation of the intervention, quality improvement, program evaluation, research, and grant writing and reporting.
  • 5% - The role will participate in related program activities including trainings, meetings, and other events. Similarly, this role supports hospital and community outreach and education to engage and educate students, healthcare providers, community members, and other stakeholders on evidence-based violence prevention and intervention strategies and trauma informed care.


Additional Knowledge, Skills, Ability Preferred:

Bachelor’s degree or current enrollment for a degree in public health, social work, psychology, health education, or other human services field, or associate degree in the same fields with relevant experience preferred. Experience in community violence prevention and intervention or trauma-informed support services preferred. Experience working with diverse, high-risk youth required. Strong verbal and written communication skills required. Experience in client/patient outreach, administering assessments, and case management preferred. Experience with Microsoft Suite (Outlook, Word, Excel) required and with electronic medical records (i.e., EPIC) strongly preferred.
