GENERAL SUMMARY

TGH Hospital at Home is seeking a motivated full time Advanced Practice Provider (APP) to join our team. Qualified applicants must have an active FL license and include Physician Assistants and nationally board-certified Acute Care Nurse Practitioners or Adult Gerontology Acute Care Nurse Practitioners.

Hospital at Home allows TGH to extend its care beyond the four walls of the health system by treating patients from the comfort of their home in the broader Tampa Bay Region. This is inpatient hospital level care delivered in the home, not home health, and not a discharge.

TGH Hospital at Home is a unique and innovative department at TGH that provides inpatient hospital level care at the patient's home with management of acute disease processes within the field of internal medicine, including, but not limited to heart failure, chronic obstructive pulmonary disease, pneumonia, urinary tract infection, cellulitis, kidney failure, surgical patients, Covid-19, DVT, sepsis, pyelonephritis, bacteremia, post liver and kidney transplants, and more. Patient care is performed in collaboration with an attending physician, medical director, and multidisciplinary team, and includes, telehealth, traveling to patients' homes, in-hospital inpatient and emergency room consults, conducting history and physical examinations, managing patients medically with possible minor bedside procedure support and facilitating interventions for patients. APPs exercise autonomous and delegated medical decision making and provide a broad range of diagnostic and therapeutic services including responsibilities for clinical coordination, education, research, and administrative services. In addition to their patient management responsibilities, APP services should include engagement in governance activities, quality / process improvement, ongoing evaluation, and program development for improved clinical outcomes. The acute care Hospital at Home APP will work closely with their Lead APP, Primary Supervising Physician, Service Physicians, Service Medical Director, The Director of Advanced Practice, Advanced Practice Leadership, Service Line Leadership and the operational team. Direct reporting is to the Lead APP and the Manager of Advance Practice Medical, Surgical and Neuroscience Services.

Qualifications

QUALIFICATIONS:

  • Nationally board-certified Acute Care NP or Adult Gerontology Acute Care NP, or a Physician Assistant.
  • Inpatient Hospital / Internal Medicine experience preferred.
  • Safe driving record.
  • Excellent communication, critical thinking, and leadership skills.
  • Ability to work effectively in a team and independently.
  • Commitment to ongoing professional development and process improvement.


WORKING CONDITIONS:

  • Work is primarily performed in-hospital via telehealth with use of computer technology for remote patient monitoring systems and virtual patient visits, in the Hospital at Home Command Center, any also can involve frequent travel between TGH facilities and to/from patient homes using TGH provided vehicles
  • On-call and weekend shifts are involved and alternated among providers
  • Daily interactions with patients, support team members, physicians, consultants, family members, and occasionally external parties (media, government officials, public).
  • Variable shifts anytime between 7am-10pm, including potential weekends.
