Job Description

PURPOSE OF POSITION

Associate Director of Nursing (ADN), Clinical and Community-Based Services will oversee a team of Transitional Care Nurses (TCNs) responsible for the coordination, management and disposition of patient referrals to the Certified Home Health Agency (CHHA). Under the supervision of the DON, Clinical and Community-Based Services, the ADN will focus on ensuring that the CHHA maintains a collaborative relationship with the acute and post- acute care facilities across the health system. The ADN will also adopt business development, and customer service strategies in developing and maintaining a close working relationship with our community and hospital-based stakeholders, to ensure their home care referral needs are met.

Duties & Responsibilities

ESSENTIAL DUTIES AND AREAS OF RESPONSIBILITIES
1. In collaboration with the DoN, Clinical and Community-Based Services, thinks strategically in planning to address the needs of referral sources, patients, and the CHHA to improve the timeliness and quality of home care service delivery.
2. Participates in short/long-range strategic planning and implements specific and targeted measures for census growth. Contributes to the development of marketing strategies and participates in programs aimed at improving the visibility of the CHHA internally and externally to the health system. Assists DoN in the creation of SWOT analyses of the respective acute care hospitals to determine opportunities for growth and strategic collaboration.
3. Strengthens relationships with new and existing referral sources by daily or weekly visits to the health system's acute and post-acute care sites. Conduct ongoing assessments to ensure TCN presence, engagement and interaction with clinical teams at their assigned h.
4. Ensures that staff are counseled appropriately and timely with regards to work performance, attendance, punctuality, quality in transitions of care, and productivity. Conducts employee performance evaluations. In collaboration with the DoN, reviews and updates current job descriptions annually, and when necessary.
5. Establishes and maintains positive and productive relationships with acute and post-acute care stakeholders, to support effective referral pathways and patient support during transition to home care. Facilitates positive interpersonal relations among staff with a focus on promoting and modeling ICARE values in all engagements.
6. Works closely with Directors of Social Work and discharge planning teams to educate patients, families and caregivers about home care regulatory guidelines, "safe" discharge planning, payer limitations and restrictions. Thinks strategically for patients and caregivers, leveraging technology to streamline referral processes and improve quality of care transitions.
7. Develops and reviews metrics that address census growth, and TCN productivity and work performance. Devises strategies to overcome barriers to meeting referral needs and drive growth of CHHA census.
8. Oversees the daily functions and activities of a team of TCNs and provides one-to-one mentoring and caching of them as needed. Assumes on-call responsibilities as needed and serves as back-up for TCN coverage.
9. Provides adequate staff education opportunities and ensures that all staff meet mandatory educational and in-service requirements in a timely manner.
10. Ensures that the CHHA is following all Federal, State, local, and accrediting body regulations with regard to management of home health referrals.
11. Conducts regular team meetings and presentations to facilitate the dissemination of information and maintain ongoing connection to the Community Care team at large.
12. Travels between multiple post-acute and acute care sites as required.

Minimum Qualifications

Qualification Requirements:
1. Licensed and currently registered to practice as a Professional Nurse in New York State, plus (2) or (3) below.
2. A Master's Degree in Nursing Administration, Education or an equivalent field and a minimum of six years of satisfactory nursing experience, of which four years shall have been in a supervisory, teaching or administrative capacity; or,
3. A Baccalaureate Degree in Nursing and seven years of satisfactory nursing experience, five years of which shall have been in a supervisory or teaching capacity; or,
4. A satisfactory equivalent combination of education and experience. However, all candidates must be licensed and currently registered to practice as a Professional Nurse in New York State.

MINIMUM KNOWLEDGE, SKILLS, AND ABILITIES REQUIREMENTS
1. Incorporates knowledge of cultural differences through effective instructional interactions. Embraces cultural competence and incorporates the same in the daily team activities.
2. Comprehends principles of adult learning and incorporates them in formal and informal educational activities.
3. Remains aware of current issues/trends in healthcare and translates them to the clinical team. Maintains "state of the art" management/professional knowledge, credentials and skill set.
4. Is able to prevent or mitigate conflict and render decisions fairly and in a timely manner.
5. Practices accountability and responsibility in all activities. Prioritizes activities, follows through, and meets deadlines.

Additional Qualifications

KNOWLEDGEABLE IN:
Demonstrated knowledge regarding Federal, State and local regulations and JCAHO standards for Home Care.
Excellent verbal, written, organizational and analytical skills.
Previous experience using various computer software applications: i.e., Outlook, Word, Excel, PowerPoint, Visio, OneNote.
