Blended Case Manager

Requirements

The ideal candidate will possess the following qualifications:

• This candidate must have one of the following:
  • Possess a Bachelor's degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education
  • OR be a Registered Nurse
  • OR possess a high school diploma AND 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science AND two years' experience in direct contact with mental health consumers required for this level.
• Must possess a valid PA driver's License and access to reliable transportation.

Earn $20.40/hour with the selection of the Enhanced Pay Option.

Position Details

Blended Case Manager (BCM) provides services in Juniata, Huntingdon, and Mifflin counties that assists children, adolescents, and adults with a variety of behavioral health/medical/community-living needs by assisting them to build a strong network of community resources, supports, and services. While meeting with clients in their homes or in community settings, the BCM will assist in linking clients with needed services in their community. A case manager will advocate for the client and their families, collaborate with them to develop an individualized service plan with measurable goals, help navigate complex systems such as special education, social services, and housing. They will assist clients and their families in maintaining and finding access to basic living needs and skills such as employment, housing, food, medical care, recreation; and they will provide support with on-call crisis intervention.

Majority of job is remote work (in the community) with designated times in the Lewistown office. Mileage reimbursement is available.
