As part of the Zero Suicide Initiative, you will play a vital role in creating safer communities by offering life-saving support and resources to individuals facing behavioral health challenges.

Position Highlights:

• 4-Year Grant Funded Role: Secure position with guaranteed funding for four years, supporting an impactful project.
• Zero Suicide Initiative: Join a cutting-edge program focused on reducing suicide rates across rural Maine through the Life Worth Living Project.
• Direct Impact: Provide critical outreach and support, guiding clients through behavioral health care and connecting them to essential community resources.
• Collaborative Team: Work alongside a dedicated team committed to error reduction, training, and improving suicide care as part of the national Zero Suicide initiative.

Essential Functions:

• Serve as a primary contact for clients and their families from the time of referral, maintaining regular communication throughout their care journey. Provide proactive support and ensure timely follow-up to ensure no one falls through the cracks.
• Guide clients in accessing essential community resources, such as mental health services, support networks, and other social services, to meet their ongoing needs and improve their well-being.
• Assist clients through various stages of care, from initial referral to active treatment and self-management. Ensure that they are connected to the appropriate services at the right time and that they understand their care plan.
• Actively contribute to the goals of the Zero Suicide program by promoting education, communication, and error reduction strategies within suicide care. Help expand the impact of MBH's efforts by providing compassionate, comprehensive support to high-risk individuals.

Required Minimum Knowledge, Skills, and Abilities (KSAs) - "What you will need":

• Education: Bachelor's degree in social work or related field preferred.
• License/Certifications: N/A

Experience: Two years of experience working with behavioral health consumers and families preferred. Experience in a team-based setting and collaborating with multiple providers preferred
