Job Description
TOUR 2: (9am-5pm)
PURPOSE OF POSITION:
To determine WIC Program eligibility.
AREAS OF RESPONSIBILITIES:
Verifies income, residential and categorical eligibility.
SUMMARY OF DUTIES AND RESPONSIBILITIES:
1. Document physical and social data in the WIC electronic files.
2. Participate in the outreach plans as required by the WIC contract.
3. Enter income and residential information in NYWIC to determine participant eligibility for WIC
services.
4. Performs anthropometric measurements for participants as needed.
5. Maintains required records and logs.
6. Maintains inventory of unit's equipment stock and supplies.
7. Distributes eWIC cards following NYSDOH guidelines.
8. Accurately document information needed by Nutritionist on the appropriate NYSDOH forms.
9. Schedule participants for appointments as necessary.
10. Operate the NYWIC system as per NYSDOH regulations.
11. All other related duties as needed and or required by the program.
Minimum Qualifications
1. High School graduation or evidence of having passed an examination for a High School Equivalency Diploma, plus three years of experience in community work or community centered activities in an area related to the duties described above; or,
2. A satisfactory equivalent.
