Summary

The Community Health Worker (CHW) will be working in the Division of Pediatrics-Public Health on a prevention grant received from the state of Texas. The CHW will be providing support to the upLIFT program, a virtual/home visitation program provided to pregnant/postpartum women with elevated symptoms of depression/anxiety. The CHW will work with staff and participants to connect families to needed services within the community. upLIFT is adding a group component to the program. The CHW will also support the upLIFT group expansion by attending and helping facilitate group sessions as needed, such as coordinating groups, sending reminders, and preparing materials.

Job Duties

  • Assists front-line staff with identification and connection to resources for participating families.
  • Coordinates upLIFT Group sessions and communicates with participants.
  • Attends and, at times, facilitates upLIFT Group sessions.
  • Prepares materials for participants for upLIFT Group.
  • Develops relationships with community organizations and facilitates a warm hand off to families.
  • Identifies and connects with community organizations for infant and maternal care supplies.
  • Develops and maintain a resource list for common goods/services needed in the grant service areas.
  • Promotes programs at community events.
  • Collects program data and entering the data into the program and state databases.
  • Coordinates with the project staff to schedule visits, send reminders, and contacts participants for follow-up as needed.
  • Helps participants connect with resources in their area and assist with applications for eligible programs.
  • Documents all activities, service plans, and results.
  • Maintains confidentiality per HIPAA guidelines in regards to patient information.
  • Attends program staff meetings.
  • Attends Division meetings.
  • Represents the Division and the upSTART at community events.


Minimum Qualifications

  • High School diploma or GED.
  • Two years of relevant experience in health/social services.


Position-Specific Requirements

  • Bilingual (English/Spanish).


Preferred Qualifications

  • Community Health Worker Certification.
