Job Description

PURPOSE OF POSITION:

The Senior Community Liaison Worker on the Mobile Crisis Unit, under supervision of licensed clinician but latitude for independent judgement, serves as members of transdisciplinary teams and provide a bridge between the hospital and the community. The Mobile Crisis Unit is geared toward reconnecting individuals to on-going community outpatient programs, thereby reducing acute psychiatric distress and the subsequent need for admission to psychiatric inpatient and/or emergency room services. Functions include serving as liaison between administration, departments, community organizations and/or other city agencies to help ensure access to appropriate services and resources available to patients throughout the hospital and within the community. The Senior Community Liaison Worker Ill is part of an interdisciplinary team on the Mobile Crisis Unit. They work alongside Licensed Independent Practitioner who is the primary evaluator of adults and children within designated catchment area of Brooklyn who was referred due to a psychiatric emergency. The program operates 7 days a week between the hours of 8am - 8pm and thus the position requires the individual work between Tours II and Ill and one day of the weekend.

AREASOF RESPONSIBILITIES:

Candidate with a B.A. in psychology, human services or health related field along with 1-3 years of experience working with the severely mentally ill, chemically addicted and intellectually disabled population will be responsible for working as part of an interdisciplinary team who is evaluating patients in the community. Candidate must have knowledge of resources and services for this population. Candidate will be required to have knowledge of and make community referrals for children and adults serviced by the Mobile Crisis Unit. They should be able to provide crisis intervention and demonstrate good de escalation techniques in the midstof a crisis. The program operates 7 days a week between the hours of 8am - 8pm and thus the position requires the individual work between Tours II and Ill and one day of the weekend. The candidate will be required to operate a hospital vehicle and thus have a valid NY State Driver's License.

SUMMARY OF DUTIES AND RESPONSIBILITIES:

The duties include working in the community of Brooklyn, visiting adults and children referred for an assessment or wellness check. The CLW goes into the community in at least a pair of2 individuals and collaborates with the clinician evaluating the patient and provide crisis interventions to high-risk patients. The CLW engages with referral source, provides support to families, and engage them as partners in recovery. They will be responsible for taking and following up on referrals received from NYC Well and within the hospital. They must have knowledge of and experience in facilitating referrals to community resources and treatment providers. Experience operating a vehicle is essential. Documenting all interventions within the required regulatory timeframes is expected. When needed, they may also be assigned to provide coverage to the CPEP by coordinating aftercare plans or transfers to other acute facilities. Good writing skills and ability to work in a fast-paced environment required and effective time management skills is necessary. English proficient a must; ability to communicate in Spanish or Creole a plus.

Minimum Qualifications
1. Four years of full-time experience in counseling, community work or community health activities in a government agency or community organization engaged in providing community services to the public, assisting members of the community in obtaining community services or maintaining liaison with schools, community organizations or other government agencies for the purpose of providing assistance and obtaining participation and support for implementation of community or public service programs; or,
2. Education and/or experience equivalent to (1 above). Study at an accredited college in Sociology, Psychology or other behavioral sciences may be substituted on a year-for-year basis for the experience described in (1 above). However, all persons must have at least one year of full-time experience as described in (1 above).
