This is a Full-Time (60 hours per pay period) with a DAY shift schedule on variable days of the week that may include weekends and holidays. Hours are typically 7am to 5:30pm.

As a Complex Care Manager, you will be responsible for the daily management and coordination of care for members with complex medical conditions such as high-risk, multiple chronic conditions, social economic, mental health, and/or substance abuse. In addition...

• Demonstrates, culturally sensitive communication and coordination with providers, hospital case management, ancillary, medical management and community resources across the system and care continuum.

• Assesses, develops, implements, coordinates, and monitors a comprehensive plan of care for each patient/family in collaboration with the physician, social worker, and all members of the interdisciplinary team.

Qualifications
Preferred Education/Experience/Specialized Skills/Certification:
  • BSN or MSN
  • Preferred in caring for the acutely ill patients with multiple disease conditions.
Required Education/Experience/Specialized Skills:
  • Associate degree in nursing
  • 3+ years of diverse clinical experience
  • 3+ years of managed care and/or case management experience.
Required Certification/Registration:
  • Current unrestricted California RN license
  • Current BLS from American Heart Association
  • Certified Case Manager (CCM) or ability to obtain CCM within 12 months of employment or equivalent case management certification and maintained during employment or 10+ years of inpatient or outpatient case management experience.
