The Complex Care Nursing Coordinator supports case managers in designated areas with complex patient discharges and is an integral part of the Case Management Department. The role reports to the Nursing Director of Case Management with primary efforts supporting patient throughput initiatives to support capacity efforts and to ensure optimal transitional care planning. The Complex Care Nursing Coordinator is responsible for researching resources, identifying, and optimizing complex discharge planning, while serving as a liaison between case managers, internal and external resources and Case Management Leadership. The role provides day to day support and guidance with complex case management and provides insight on process improvement and opportunities to improve and enhance throughput initiatives. The role works closely with the Capacity Case Manager to ensure timely and appropriate transitions in care occur and alternate solutions and plans to discharge are thoroughly explored.

  • Acts as a case manager expert and mentor to identify and resolve issues impacting quality, safety and timeliness of care transitions and escalates to appropriate stakeholders as appropriate
  • Co-manages with unit-based case managers complex patient care discharges and collaborates with relevant clinical teams both internally and externally.
  • Identifies potential discharge barrier mitigation strategies to ensure care progression and safe care transitions
  • Identifies new escalation pathways and shares with case management team, providing valuable resources and updates
  • Responsible for executing and enforcing best-practices, and new practices (e.g., alternate care pathways) related to discharge planning.
  • Provides mentorship and guidance to clinical teams and case management as needed related to complex care and post-acute discharge
  • Monitors ongoing performance of initiatives and develops recommendations/identifies opportunities for performance improvement
  • Analyzes trends in LOS, census, readmissions, and non-qualified hospital days and the impact on continuum-based utilization and outcome
  • Coordinates efforts to optimize hospital days, including ineffective admission determinations, discharge planning, utilization of ancillary services and timely placement
  • Works closely with the Capacity Center to identify opportunities to enhance throughput efforts and optimize discharge options


Qualifications

  • RN with current license to practice in Massachusetts required.
  • Bachelor's degree required; master's degree preferred.
  • Case Manager certificate preferred.
  • 5 + years of inpatient case management experience with advanced knowledge around standards of practice and regulatory and payer compliance required.
  • Extensive experience and knowledge in discharge planning required.
  • Extensive understanding of post-acute resources required.
  • Utilization Review experience preferred.
  • Case Management in acute setting and Utilization Management experience/knowledge
  • Strong assessment and problem-solving skills
  • Strong interpersonal skills
  • Ability to work independently with minimal supervision.
  • Goal oriented and accountable
  • Demonstrated organizational skills.
  • Demonstrated ability to work in a complex setting.
  • Ability to work in an interdisciplinary team-based environment.
  • Strong oral and written communication skills
  • CM Certification preferred.
  • Computer experience preferred.
