MDS Nurse Coordinator/Case Manager (RN/ LPN)

Due to growth in census, Brighton House Rehabilitation & Nursing Center is seeking an ON-SITE, full-time MDS Nurse Coordinator/Case Manager (RN or LPN) to support our clinical reimbursement team. Requirements for MDS Nurse Coordinator/Case Manager (RN/ LPN) :
  • Experience in MDS completion required
  • Certification as a Resident Assessment Coordinator (RAC-CT) preferred
  • Ability to monitor, evaluate, and manage plans of care for in-house residents and patients
  • Excellent communication skills with residents, families and the interdisciplinary team
  • Strong computer skills, including Microsoft Office, EMR systems, etc.
  • 1-2 years experience in long term care
  • Active, unencumbered licensure as a registered nurse (RN) or licensed practical nurse (LPN) in the state of MA
  • Prior experience with PointClickCare (PCC) required.
  • Experience with PointRight (analytical care management) software, preferred.

Job Duties for MDS Nurse Coordinator/Case Manager (RN/ LPN)
  • Assess and monitor proper treatment for residents in nursing homes .
  • Determine the health status, care plans, and procedures for intake of residents according to state and federal standards.
  • Perform clinical assessments
