JOB PURPOSE:

The primary purpose of the Director of Care Coordination and Patient Experience is to support the facility level Senior Care

Partners in ensuring that residents within the organizations skilled nursing facilities receive high-quality, coordinated care that meets their individual needs while simultaneously enhancing their overall experience.

The Director of Care Coordination and Patient Experience aims to create a standardized, patient-centered approach across all facilities, driving excellence in care delivery, patient satisfaction and operational efficiency.

The Director of Care Coordination and Patient Experience through leadership and support will foster a culture of empathy, respect, and continuous improvement, ensuring compliance with regulatory standards and achieving organizational goals for resident care and satisfaction.

KEY RESPONSIBILITIES:

1. Develop and execute comprehensive care coordination and patient experience strategies aligned with corporate goals.

2. Standardize care coordinating protocols and patient experience initiatives across all facilities.

3. Identify and implement best practices for resident care and satisfaction.

4. Provide leadership and guidance to nurse navigators assigned as Senior Care Partners.

5. Foster a culture of excellence in resident care and satisfaction

6. Ensure all facilities comply with relevant regulations, standards, and guidelines.

7. Develop, revise, and review policies and procedures to maintain high-quality care.

8. Analyze data and trends to identify areas for improvement and implement corrective actions.

9. Design and implement organizational initiatives to promote resident satisfaction and overall experience.

10. Conduct scheduled audits and assessments to gather feedback and identify areas for improvement.

11. Monitor and evaluate the effectiveness of care coordination efforts. Identifying barriers to effective care coordination and assisting in addressing as needed.

12. Serve as a liaison between corporate leadership, regional directors, and facility staff.

13. Promote a collaborative environment that supports interdisciplinary teamwork.

14. Represent the organization in forums and conferences.

15. All other duties assigned.

KNOWLEDGE, SKILLS, ABILITIES:

• Comprehensive understanding of healthcare delivery systems, particularly in long-term care setting

• Knowledge of care coordination and case management principle

• Familiarity with resident experience best practice

• Understanding of interdisciplinary team dynamics and collaboration strategie

• Working knowledge of Microsoft Word, excel, outlook and Electronic Health Records- MatrixCare

• Ability to organize and prioritize workload

• Knowledge of disease processes, clinical pathways, medications and side effects.

• Must be an effective communicator

• Ability to collaborate and coordinate individualized resident centered care

• Understanding of the discharge planning process, resources and healthcare service line

• Understanding of the Case Mix Meeting proce

• Understanding of the Post Admission Care Conference

• Understanding of the Quality Assurance Performance Improvement Proce

• Knowledge of CMS guidelines/regulations as they pertain to this position

Qualifications:

MINIMUM EDUCATION REQUIRED:

Graduate of an accredited educational institution with a degree in Nursing

MINIMUM EXPERIENCE REQUIRED:

Minimum of 3 years of experience in long-term care or similar healthcare environment

MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:

• Must possess an active Registered Professional Nursing License

• CPR Certification

ADDITIONAL QUALIFICATIONS: (Preferred qualifications)

• Ability to analyze data, identify trends and implement improvement strategie

• Strong understanding of care coordination, patient experience and quality improvement principle

• Ability to lead and manage teams across multiple location

• Communication, leadership, and interpersonal skill
