Facility: Cypress Garden Center for Nursing & Rehab
Location: Flushing, NY
Salary: $85,000 - $95,000

We are seeking a dynamic Director of Social Services to lead and manage our Social Services Department. In this key leadership role, you will be responsible for planning, organizing, and overseeing the department's operations to support residents and their families.

Responsibilities:

• Conduct patient assessments, develop personalized care plans, and assist patients and families in adjusting to their new environment.
• Lead the Social Services Department, setting goals and policies to ensure exceptional service.
• Collaborate with other departments to address and resolve complex situations as they arise.
• Regularly assess residents to ensure their evolving needs are met, and keep families informed about ongoing care plans.
• Engage residents and families in shaping social services programs whenever possible.
• Provide solutions for a range of social and practical challenges, such as financial assistance, discharge planning, and community referrals for specialized support.
• Collaborate with administration, medical, and nursing teams, participating in facility surveys and inspections.
• Advocate for residents by interpreting their social, emotional, and psychological needs to the medical staff and care team.

Requirements:

• A minimum of 3 years of experience working in a Skilled Nursing Facility (SNF).
