Job Description

Provide patient care services to patients in Behavioral Health. To provide the patients with risk assessments, mental health services and ensures safe discharge planning back into the community

• Complete psychosocial assessments on all new admissions to determine patient's healthcare needs
• Formulate and initiate appropriate discharge planning options. Responsible for all documentation that will facilitate patient's discharge
• Facilitate and organize outpatient care with other providers
• Be an active part of the clinical team, arrange family meetings and provide family and individual therapy in inpatient settings
• Evaluates the patient's behavior with sensitivity to the patient's lifestyle and culture. Interviews the patient's family/ parent/ significant other to understand why situation (s) is/are regarded as problematic. Perceived symbolic and distorted meanings in aptitudes and behavior

Minimum Qualifications

For Assignment to Level II:
1. Valid license as an LMSW issued by the NYSED; or
2. Valid license as an LCSW issued by the NYSED.

Salary may differ based on the licensure (LMSW or LCSW) of the candidate.
