Job Description

Social Worker V provides supervision and support to DV Shelter caseworker and social workers and to H+H Level 2-4 Social Workers in the screening, assessment and referral of clients for mental health services by Heath and Hospital clinical team. Social Worker V provides care and coordinates services at a designated shelter location, the Family Justice Centers, or via Tele-Health.

Under general direction, with very wide latitude for independent judgment and decision making, in addition to performing the duties of the lower Assignment Levels, performs the following typical tasks:
  • Provides professional clinical supervision through review of cases to assist and guide subordinate social service staff in assessment, referrals and treatments of DV shelter clients.
  • Provides adult, children, and group psychotherapy including crisis and grief counseling at required level.
  • Conducts intake assessment for all assigned DV referrals to determine care needs
  • Collaborate with center staff and on-site service agencies related to clients' clinical needs. Assist in facilitating referral for psychiatric care when appropriate.
  • Supervises and mentors assigned SW II-IVs and peer counselors.
  • Performs administrative duties including monitoring care metrics to ensure we are meeting Central Office targets.
  • Monitors all patients in case load from referral to discharge to ensure good patient care
  • Present intake data at weekly clinical meeting.
  • Maintains and completes statistics of case activities as outlined or requested by Associate or Assistant Director.
  • Provide trauma informed education and prevention to DV shelter staff as requested by Director.
  • Determines work assignments to support DV providers across assigned shelter sites, coordinates work flow and duty assignments, and evaluates work performance of subordinate social service staff.
  • Ensures patient records are stored securely, confidentially, and compliant with HIPAA standards. Interfaces with hospital Medical Records department as necessary.
  • Attend all trainings and departmental meeting as requested.
  • In the temporary absence of the supervisor may perform duties if requested.


Minimum Qualifications
1. Valid license as an LCSW issued by the NYSED, and two (2) years of supervisory experience, as described above.

Department Preferences

  • Three years background at responsible management and administrative levels; or, An equivalent combination of training, educational background and experience in related fields and educational disciplines; and General knowledge of policies and programs utilized in the operation of a health care facility, ability to assimilate and analyze data, to recommend action based on analytical findings,and to assist in the solution of complex problems in health care administration and management

• Section 424-a of the New York Social Services Law requires an authorized facility to inquire whether a candidate for employment with child-care responsibilities has been the subject of a child abuse and maltreatment report. The facility has the discretion to assign a candidate who has been the subject of a child abuse and maltreatment report to a position with no child-care responsibilities.

KNOWLEDGEABLE IN:
  • Experience with Domestic Violence and/or trauma population. Strong interpersonal and communication skills and ability to work well in a multidisciplinary team setting. Familiarity with high-risk patients and cultural sensitivity in community settings. Familiarity with high-risk populations and community settings, demonstrated interest in maternal mental health. Administrative and research experience a plus. Child treatment also plus.
  • Three (3) years' experience in a supervisory setting, preferably in Behavioral Health Services
  • Supervises and trains subordinate social workers and support staff
  • Develops instructional materials and techniques
  • Bilingual Spanish preferred. Other languages a plus.
