Responsibilities

Provides individualized self-management education and training to individuals and groups according to the scope of practice. Group Leaders maintain a non-technical role in providing instruction to program participants. This position requires full understanding and active participation in fulfilling the mission of the organization. It is expected that the employee will demonstrate behavior consistent with the core values.
  • Able to lead, facilitate and manage groups of students and participants in Community Health programs with effective communications skills and professionalism.
  • Utilize evidence based practices as indicated in curriculum being presented for each program and maintain fidelity of each program.
  • Develop a collaborative, therapeutic alliance with participants and community members.
  • Identify when a program participant's needs are outside the scope of the group leader's expertise and has the ability to arrange for additional services to meet needs.
  • Provide outreach to older adults and work with community based groups. Make oral presentations on health topics and programs details in community settings.

Qualifications

Minimum:

  • Experience in community health education, clinic or acute care or, promotora or community health worker , health and wellness, life style coach or support group facilitator.
  • Must possess and maintain a current Basic Life Support (BLS) Certificate.
  • Must possess and maintain a valid California Driver's license and proof of auto insurance.
  • Must possess and maintain leader certification in one or all of the following: CDSMP, DSMP, Tomando Control de Su Salud, Manejo Personal de la Diabetes, Walk With Ease, Matter of Balance and DPP or maintain certification within 1 year of hire.

Preferred:
  • Prior medical field experience and knowledge of medical terminology
  • Bilingual in English and Spanish
