POSITION SUMMARY:
In alignment with Health First commitments of Quality/No Harm, Customer Service and Stewardship the Hospital at Home (HaH) RN Coordinator will ensure optimal hospital level clinical care and the provision of an excellent customer experience. The RN Coordinator will collaborate with the clinical team to ensure a comprehensive plan for patients, with complex and chronic care needs, including but not limited to, identification of patients meeting inclusion and exclusion criteria for the program, ensuring appropriate plan of care is in place and executed, and working with a multidisciplinary team to ensure that all resources are available to deliver optimal patient care across the care continuum. The HaH RN Coordinator will serve as an educational resource for patients, care givers and staff members at patient's bedside, the patient's home and/or telephonically. PRIMARY ACCOUNTABILITIES:
1. Identify appropriate patients meeting HaH criteria, in coordination with the Emergency Department team, as determined by need to best serve the patient.
2. Facilitate patient's transitional plan in collaboration with the physician, nursing and interdisciplinary team.
3. Facilitate collaborative management of patient care across the continuum, intervening as necessary to remove and escalate barriers to ensure timely and efficient care delivery.
4. Facilitate and promote high quality, cost-effective outcomes, focused on the whole patient orientation and self-management decision support and to minimize any fragmentation of health care delivery.
5. In conjunction with the manager, provide appropriate delegation and supervision of patient care activities; monitor patient outcomes to meet or exceed regulatory compliance.
6. Provide service, education and resources to patients and families with sensitivity and respect for their needs, expectations, age, cultural, and individual differences. Respond appropriately to unexpected, stressful situations while professionally interacting with all, including co-workers.
7. Monitor and ensure accurate, timely records from admission to discharge. Work collaboratively throughout the systems to access and update information, and to support the delivery of quality service.
8. Communicate timely updates with interdisciplinary team during team huddles, including other members of the healthcare team, to promote seamless, quality delivery of care.
9. Model accountability and other expected behaviors in all aspects of job performance for self, the team and department success. Includes flexing own workload and schedule to serve patients as needed.
10.Collaborate with the hospital's Quality department to monitor outcomes to meet regulatory requirements, utilizing quality software programs for all data. Promote quality patient care and customer service/satisfaction, with safe, cost efficient measures, and a commitment to process improvement.
11.Provide patient, family, and/or caregiver education as directed by the plan of care; promote healthy behaviors as much as possible.
12.Help or assist others, i.e., new and existing team members, on department specific procedures and applications.
Qualifications:
MINIMUM QUALIFICATIONS
Education: Associate Degree in Nursing or Nursing Diploma
Work Experience: One (1) year Acute care experience
Licensure: Current Florida RN license
Certification: Current American Heart Association BLS Healthcare Provider; eligible Level II background screening
Knowledge/Skills/Abilities: o Clinical competency, i.e., clinical assessment, medication administration, other clinical skills required and as needed for role o Microsoft Office including excel, word, PowerPoint and electronic medical record (EMR) o Time management; Prioritize multiple tasks; meet deadlines o Initiative; work independently o Critical thinking; sound judgment; decision making o Customer service o Teamwork o Adaptable; Flexible - workload and work schedule changes o Self-awareness; emotional intelligence
PREFERRED QUALIFICATIONS:
Education: BSN
Work Experience: One (1) year in Acute Care experience and One (1) year in Home Care/ Private Duty nursing

Knowledge/Skills/Abilities: o EMR applicable to current hospital system o Facilitation skills
PHYSICAL DEMANDS:
o Travel to/from various Health First locations and patient's residence o Lift a minimum of twenty (20) pounds unassisted, up to five (5) times a day o Able to assist patients with ADLs, transfer and ambulation o Frequently bend, stoop, walk, kneel; occasionally sit o Must be able to tolerate a minimum of 3-5 hours of computer time a day
