We are currently seeking a full time LVN Client Care Coordinator (CCC).

The LVN Client Care Coordinator is responsible for creating extraordinary client and family experiences and ultimately manages "best in home care" care plans.

  • Develops and maintains the client care plan, creating extraordinary client experiences and communicating with clients, families, care partners and referral sources.
  • Plays an integral role in interfacing with clients and families to ensure safe, appropriate and meaningful care is being provided.
  • Serves as a mentor to caregivers and supports their care delivery.
  • Maintains a calm, positive demeanor at all times and is able to effectively multitask in a high functioning office environment.
  • Evaluates clients status on a 60-day cycle, reviews and coordinatoes care plan with client needs.
  • Evaluates the appropriate assignment of personnel to meet the specific need of individual clients and coordinates with office staff.
  • Ensures that changes in the client's medical or mental condition are reported to the client's damily, healthcare professionals and to the Director of Operations.
  • Investigates problems, irregularities, reported incidents, policy violations and other reportable events implementing the appropriate corrective action and follow-up.
  • Maintains a calm, positive demeanor at all times and is able to effectively multitask in a high functioning office environment.
  • The ideal candidate has prior experience working in the home care industry or medical office setting.
  • Participates in after-hours call rotation.
  • Promotes agency to the community, participating in commuity programs with other professionals when possiable.

The desired candidate will possess the following:
  • Graduate of a State approved school of vocation nursing program with 6 months of satisfactory nursing experience.
  • Currently licensed without encumbrances and in good stafning as an LVN in the state of California.
  • Able to work independently, demonstrating sound judgment.
  • Be available as required for on-call duty outside of normal office hours.
  • Have a valid driver's license and use of insured automobile.
