Now Hiring: Medical Care Coordinator

Responsibilities:

• Patient Liaison for the residents and families
• Care Plan Development for residents by evaluating their current state and determining the action plan to healing.
• Assist residents in accessing necessary healthcare resources, including medications, medical equipment, and support services.
• Build relationships with local organizations, businesses, and government agencies to strengthen support networks.
• Engage in community outreach efforts to raise awareness of shelter services and resources.

Qualities:

• Knowledge of medical terminology
• Must be a Licensed Vocational Nurse in CA
• Experience with the underserved population
• Compassionate towards humans
• Experience in the medical field eg. EMT, Paramedic a plus
