Job Summary:
This is for PRN only to work at our three community health centers. As a member of the Family Practitioner Team, participants in the diagnosis, treatment and supportive care of the appropriate age group and any others as defined by the Collaborative agreement the NP has with the collaborating physician.

This position is involved in a team-based approach to care. Team members are trained to meet the highest level of function for their role as per the State of Tennessee/Georgia guidelines. Is trained and assigned (as per their role and responsibilities) to coordinate care for patients and is trained and assigned to support patients/families/caregivers in self-management, self-efficacy and behavior change. Is trained and assigned (as per their role and responsibilities) to manage the practices' patient population. Participates in the practice's quality improvement process and performance evaluation.

Education:
Required:
RN with a Master's degree in nursing certification in family and/or adult nurse practitioner. Graduate of a nationally accredited school of nursing.

Preferred:

Experience:
Required:
One or 2 years of RN experience.

Preferred:
Minimum of three years experience in a physician office setting

Required:
Current licensure to practice nursing in the State of Tennessee.

The Nurse Licensure Compact will not change how to obtain or renew a Tennessee license. However, the Tennessee nursing license will be a single state license for Tennessee Residents or non-compact state residents. Tennessee licensure or multistate licensure from a compact state must be obtained within three months of hire for non-Tennessee residents.

Preferred:
Certification in areas of specialty preferred.

Department Position Summary:
A nurse practitioner in the Community Health Center will work under direction of the physician/medical director. He/She will be responsible for developing and utilizing an individualized plan of care. He/She must be able to collect, document, and analyze patient data. They must be able to collaborate with the health care team in the development and implementation of testing and treatment according to protocols. He/She must be able to participate in health screening programs.

The individual must demonstrate knowledge of the principles of growth and development over their life span and possess the ability to assess data reflective of the patient's status. He/She must interpret the appropriate information needed to identify each patient's requirements relative to his or her age-specific needs and to provide the care needed as described in the unit's policies and procedures. This is done through evaluation of direct supervisor by direct observation, in a skills lab, or discussed verbally or written. Competency can also be evaluated by a formal peer review process and/or by self assessment based on objective criteria that is reviewed and validated.
