Job Description

Purpose:

Under the general administration of the Chief of Ambulatory Care, collaborates with and is accountable to the Collaborating Physician for health care assessment. Identifies, diagnoses and treats human responses for actual or potential health care problems for individuals and families. Provides such services as case-finding, health counseling, implementation and evaluation of primary health care in the area of infectious disease care; provision of care supportive to or restorative to life and well-being; and the selection of performance of those therapeutic measures essential to effective management of the nursing regimen and the execution of any prescribed medical regimen.

Specific Duties and Responsibilities:

The Nurse Practitioner is charged with and responsible for:

  • Work in partnership with Ambulatory care leadership to lead ID & HIV care initiatives
  • Counsel patients on benefit education as it relates to HARP & SNP programs in collaboration with managed care team.
  • Primary site lead participating & coordinating with central office on all matters as it relates to primary HIV care.
  • Provide PrEP & PEP counseling as well as treatment management.
  • Work in partnership with collaborating physician/support team on all state reportable events.
  • Perform History & Physical examinations.
  • Order routine diagnostic testing, e.g., laboratory, radiologic, etc.
  • Ongoing medical documentation in the medical record, e.g., progress note, etc.
  • Refer for appropriate consultation (s).
  • Order medications and write prescriptions subject to Section 6902 (3) (b) of the New York State Education Law.
  • Education and counseling of patients and families.
  • Perform more advanced procedures and BCLS with documentation of appropriate training and documentation of competency.
  • Provides to patients and families information relating to the application of health status, treatment and progress. Counsels and guides patients and families in order to develop favorable attitudes toward health care and maintenance, and to encourage practices that promote health and well-being and reduce anxiety, stress and disability.
  • Other duties as required under license.


Minimum Qualifications
1. A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED); and
2. Current certificate as a Nurse Practitioner issued by the NYSED; and
3. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
