The Virginia Department of Health (VDH) is dedicated to protecting and promoting the health of Virginia. The VDH is made up a statewide Central Office in Richmond and 35 local health districts.

As part of the Virginia Department of Health, Hampton, and Peninsula Health Districts (HPHD) are two health districts serving the residents of the Cities of Hampton, Newport News, Poquoson, Williamsburg, and the Counties of James City, and York.

As the Population Health Community Coordinator, you'll play a key role in advancing health equity by leading and coordinating targeted health programs. You'll work alongside the Population Health Manager to oversee the planning and development of initiatives that address the unique needs of our community, while also managing a team of Community Health Workers. Strong communication skills are essential, as you'll collaborate with community partners, engage stakeholders, and promote our health initiatives. You'll also analyze data to identify health trends and measure the impact of our programs, ensuring we continuously improve our efforts. The Population Health Community Coordinator serves as the Healthy Communities Coordinator for the local health district's efforts to work on developing and implementing environmental strategies to improve population health. Represents the health district at community events and assists, as necessary, in the coordination of health community activities. Supports and reinforces healthy behaviors in the district by working with community coalitions, schools, businesses, city leadership, urban designers and other local organizations and partners to build support for lifestyle change. Independently seeks out grant opportunities and contacts potential community partners.

Independently develops and presents health education materials and programs, coordinating schedule and overarching message with supervisor and district leadership team. Seeks guidance on development of new or non-routine projects and seeks approval before committing resources to a project or program.

Minimum Qualifications

•Knowledge of and ability to: collaborate with a diverse group of community partners; experience planning, implementing, and evaluating public health programs; excellent oral and written communication skills; proven ability to work independently

• Ability to use Microsoft Word, Excel, PowerPoint and Gmail; ability to manage social media platforms; Ability to integrate knowledge of socioeconomic and behavioral determinants of health in chronic disease prevention work

• Knowledge of CHA/CHIP activities; experience collecting, analyzing, and presenting data to drive activities and prioritize needs; experience managing a program budget; experience integrating principles of cultural sensitivity and appropriateness in chronic disease-related health promotion work; experience in grant writing and management; experience in teaching chronic disease prevention/ health promotion; experience utilizing evidence-based strategies to improve population health; and experience utilizing policy, systems, and environmental strategies
