Summary:
Has the strong clinical expertise and deep compassion and sensitivity it takes to build a one-on-one relationship to support patients receiving infusion therapy in the comfort of their homes while also balancing a fast-paced, dynamic work environment. Works collaboratively as part of the Home Infusion team with the health system and referral sources to coordinate home infusion services, provide training for patients and caregivers, and perform the functions of a Registered Nurse in the clinical and home care settings. Establishes and manages the Home Infusion Nursing Department, leading the process of obtaining Home Health Agency (HHA) licensure and accreditation, ensuring compliance with all regulatory requirements, and building a team of skilled nursing professionals to deliver specialized, compassionate infusion-related home care services.
Formal Education Required:
a. Graduate of an accredited school of professional nursing or a school accepted for candidacy status from an accrediting body.
b. Bachelor's degree in nursing preferred.
c. Current license to practice registered nursing in the State of Ohio.
Experience & Training Required:
a. Two (2) years' experience in care coordination in a managed care environment and/or homecare or community agency.
b. One (1) year of project management or team leader experience preferred.
Other Skills, Competencies and Qualifications:
a. Case Management or Utilization Review certifications preferred.
b. Ability to travel from location to location throughout the workday.
c. Population Specific Competency: Ability to effectively interact with populations of patients/customers with an understanding of their needs for self-respect and dignity.
d. Demonstrates knowledge of healthcare-managed care, the care continuum, and the transition of patients along the various levels of care.
e. Attentiveness to detail.
f. Supervisory skills.
g. Ability to prioritize duties and delegate responsibilities.
h. Excellence in written and verbal communication.
i. Competent and comfortable working in different environments such as hospitals, outpatient settings, offices, and patient homes, which present numerous interruptions while requiring attention to multiple simultaneous tasks.
j. Ability to effectively interact with populations of patients/customers with an understanding of their needs for self-respect and dignity.
Level of Physical Demands:
a. Ability to safely lift and manipulate equipment, containers of product and supplies, etc., frequently encountered in home infusion and home care settings.
b. Hepatitis B vaccination or signed statement of declination of the vaccine.
c. Must be ambulatory and able to drive to patients' homes, able to climb stairs, pull, push, and facilitate movement of the patient.
d. Must be able to read, write and communicate with patients.
e. Must have correctable vision and correctable hearing to be able to hear normal conversations. Must be able to work in settings with infectious diseases.
f. Heavy Work - Exerting 50 to 100 pounds of force occasionally, and/or 25 to 50 pounds of force frequently, and/or 10 to 20 pounds of force constantly to move objects. Physical Demand requirements are in excess of those for Medium Work.
