Why does the job exist?

This position is a Service Coordinator who works with the external Case Manager and all members of the Interdisciplinary Team to assure the presence and availability of services and provides support for persons with intellectual and developmental disabilities receiving Living Supports from Los Lunas Community Program (LLCP). The Service Coordinator will be responsible for managing a caseload of individuals receiving services via the DD Waiver, State General Funds, Medicaid, or other funding sources. Position is responsible for knowledge of DD Waiver Standards and completing all tasks related to the position. Position requires attendance at all required meetings, training staff on required documentation, submitting DD Waiver budgets and required documentation, and maintaining Medicaid, Social Security and other paperwork. Position may include possible duties related to requirements of a Qualified Intellectual Disabilities Professional (QIDP) for persons served in our Intermediate Care Facility (ICF). Position is responsible for admissions and discharge planning of individuals, auditing and updating individuals¿ files, monitoring and implementing Individualized Service Plans (ISPs) including plans for Supported Living, Customized Community Supports (CCS), and Community Integrated Employment. The Position also participates in and promotes agency CCS initiatives and opportunities. Position responsible for maintaining professional relationships and communications with all team members and co-workers. Service coordinators are also required to work in the field completing house visits. Position responsible for following all NM Department of Health and LLCP Policies and Procedures.

Service coordination includes working with internal and external interdisciplinary team members to advocate for and coordinate appropriate and effective services for persons receiving Supported Living, Customized Community Supports, and Community Integrated Employment services.

Who are the customers?

Our customers include adults with intellectual and developmental disabilities (IDD) enrolled in our programs, members of their interdisciplinary support teams and their guardians, all Los Lunas Community Program (LLCP) staff, and other stakeholders. LLCP serves a diverse group of persons with IDD including some persons with acute behavioral and/or medical needs.

Ideal Candidate

Experience working with persons with intellectual and developmental disabilities (IDD).
Knowledge of New Mexico Medicaid Developmental Disabilities Waiver standards and Home and Community Based services.
Knowledge of DD Waiver budgets and standards.
Experience working with interdisciplinary teams.

Minimum Qualification

Bachelor's Degree in Social Work, Psychology, Guidance and Counseling, Education, Sociology, Criminal Justice, Criminology and/or Family Studies/Services and two (2) years of any combination of experience including working with communities, working on health or social service related matters, social work/case management experience, behavioral health and/or health care. Employment Requirements

Must possess and maintain a valid Driver's License. Must possess and maintain a current Defensive Driving Course Certificate from the State of New Mexico or must pass and receive a Defensive Driving Course Certificate as a condition of continued employment within 45 days of employment. Successful completion of the Health Insurance Portability and Accountability Act (HIPAA). Employment is subject to a criminal background check and is conditional pending results.

Working Conditions

Office setting with extensive use of personal computer, telephone, fax, copier and other related office equipment that involves sitting, standing, bending, reaching, etc. to include lifting up to 30 pounds. Some in-state travel is required. Occasional overtime may be required. Incumbent is subject to individuals who display violent and behavioral outbursts.
