The Aging and Long-Term Services Department provides accessible, integrated services to older adults, adults with disabilities, and caregivers to assist them in maintaining their independence, dignity, autonomy, health, safety, and economic well-being, thereby empowering them to live on their own terms in their own communities as productively as possible. Adult Protective Services investigations collect and assesses information to determine whether the reported incident of adult abuse, neglect, exploitation, or self-neglect occurred. Investigations assess the immediate safety of the adult and the ability of the individual to self-protect and/or the caregiver¿s ability to protect the adult; also, to assess whether the adult remains at risk for continued abuse, neglect, or exploitation and self-neglect. Finally assess the need for services, including short-term protective services.

This position exists to assist with project coordination for the Office of Alzheimer's and Dementia Care (OADC) and the New MexiCare Program, which provides statewide comprehensive social work services.

Assists with program coordination for the New MexiCare Program and Office of Alzheimer¿s and Dementia Care (OADC), which provides statewide comprehensive social work services.
Assists with project development associated with the New Mexico State Plan on Alzheimer¿s Disease and Other Dementias.
Assists with support of the OADC Advisory Council.
Provides client advocacy as appropriate.
Attend statewide trainings and conferences.
Will login and manage incoming calls for the program, assist with referrals and inquiries on services offered.
Conducts client/caregiver screenings, intake assessments and ongoing monitoring.
Provides case management and support to clients seeking caregiver services and resources.
Provides care navigation by helping to empower clients and caregivers in navigating health and social service systems.
Assists clients with care plan and safety plan formulation.
Uses independent judgment to provide social work services to the public. Actively seeks out and participates in appropriate clinical supervision.
Assists with development and implementation of departmental and programmatic policies and procedures.
Assists with planning, developing and evaluating program delivery of a broad scope, coordinating with internal agencies and external agencies.
Assists in building relationships with both internal and external agencies in order to leverage services.
Participates in quality improvement activities.
Assists with providing in-service training to both internal departments and external organizations and groups in accordance with agency policies and procedures.
Assists with data collection and reporting to help identify gaps in services and improve services statewide.
Assists with the preparation of reports, spreadsheets, invoice payments.
Assists in providing program outreach on all services and programs offered. Outreach activities may include evening, weekend and statewide travel.
Works with colleges, universities, and stakeholders, and participates in program committees, and other groups as assigned.

Ideal Candidate

The ideal candidate must be professional, courteous, civil, respectful and must provide open, honest and thorough communication. Strong communication skills with partnered entities for program
success. This position would be someone who can multi-task, work autonomously, and provide outstanding customer service to a wide range of individuals. The person must demonstrate strong
critical thinking, case management, working with diverse individuals, working in the field in clients' homes that may have health or safety concerns, conducting assessments, completing
documentation, and ensuring all reporting is accurate and timely. This position requires travel and overnight stay.
* Experience managing staff of high-volume case levels, offer guidance and direction to staff.
* Experience in program management, development for program success. Experience coaching staff on how to effectively respond to clients who may be confrontational, troubleshoot issues or
concerns on interoffice or on field.
* Experience coordinating services for vulnerable clients or persons with disabilities, Veterans, and adult population.

Minimum Qualification

Bachelor's degree in social work, Psychology, Guidance and Counseling, Education, Sociology, Criminal Justice, Criminology and/or Family Studies/Services and four (4) years of any combination of experience including working with communities, working on health or social service-related matters, social work/case management experience, behavioral health and/or health care. Employment Requirements

Must possess and maintain a valid driver's license and must have own mode of transportation for daily client-related travel. Personal vehicle and operation of it must be in compliance with all applicable State of New Mexico statutes and regulations. Preemployment background investigation is required, and extensive statewide travel is required. Must complete Defensive Driving Course.
Must pass State and Federal Criminal Background Check.

Working Conditions

Work is performed in an office setting: late hours and weekends work may be required. Will be exposed to regular periods of video display terminal and keyboard usage and stressful situations. Travel may be required. Incumbent will work under stress and frequent time constraints.
