Why does the job exist?

The Social & Community Service Coordinator - Operational/Community Inclusion Coordinator will:
  • Relay information about day services and employment services to individuals on the DD waiver, MI Via Waiver, Supports Waiver and State General Fund Waiver;
  • Provide support to those individuals receiving Day and Employment Services and to support those Agencies that provide Day and Employment Services;
  • Provide technical assistance and support to many staff that have not provided day services or supported employment;
  • Present the right to work at ISP meetings or IDT meetings.


How does it get done?

The Social & Community Service Coordinator - Operational/Community Inclusion Coordinator is responsible for:


  • Attending interdisciplinary team meetings;

  • Assisting the team with problem solving and mediating conflict resolution within the team;

  • Monitoring service provider compliance and follow up on the need to support employment and meaningful daily activities in the Northeast Region;

  • Communicating with teams, providers, family members, guardians, advocates and individuals that are inquiring about providing a meaningful day for every person they serve, to include the right to work;

  • Ensuring the mandate for site visits;

  • Ensuring the NERO completes over 750 visits every 6 months as recommended by the Governor;

  • Participating as part of the site visit team;

  • Traveling as needed (a minimum of one (1) day per week).

    Who are the customers?

    This position assists with people receiving services through the DD Waiver.
    The site visits will include Mi Via, Supports Waivers and Medically Fragile people as well as people transitioning between waivers.
    The site visits are conducted to ensure home safety and health of the person.

    Ideal Candidate

    The ideal candidate has experience:
    • Assisting Case Managers, Providers, and Therapists regarding the Regulations, DDW Service Standards, and Policies and Procedures for the Developmental Disabilities Waiver, Mi Via Waiver, Support Waiver, Medically Fragile and State General Fund programs;
    • Prioritizing numerous competing tight deadlines and will have strong organizational skills;
    • With a strong work ethic and good critical thinking and communication skills;
    • Providing same day travel to areas int he Northeast;
    • Entering the homes of people they do not know and asking health and safety questions.


    Minimum Qualification

    Bachelor's Degree in Social Work, Psychology, Guidance and Counseling, Education, Sociology, Criminal Justice, Criminology and/or Family Studies/Services and two (2) years of any combination of experience including working with communities, working on health or social service related matters, social work/case management experience, behavioral health and/or health care.     Employment Requirements

    Must possess and maintain a valid New Mexico Driver's License. Must possess and maintain a current Defensive Driving Course Certificate from the State of New Mexico or must pass and receive a Defensive Driving Course Certificate as a condition of continued employment within 45 days of employment. Travel may be required with some occasional overnight stays. Successful completion of the Health Insurance Portability and Accountability Act (HIPAA)

    Working Conditions

    Office setting with extensive use of personal computer, telephone, fax, copier and other related office equipment that involves sitting, standing, bending, reaching, etc. to include lifting up to 30 pounds.
