Job Description

TOUR 2 (8am - 4pm):

PURPOSE OF POSITION:

The Social worker III will provide clinical supervision to the social workers of the mental health continuum program
The Social Worker III and II position is located in the Behavioral Health Department and is a member of an interdisciplinary treatment team and, in coordination with the team, provides treatment children according to OMH, Network and the Profession's standards. Through the Mental Health Continuum program, the Social Worker III and II will support the behavioral health needs of students at the selected schools in Brooklyn. They function as a therapist in the Outpatient Mental Health Clinic serving children and adolescents with mental health conditions and co-morbid medical and substance use disorders. The goal is to provide meaningful and stable connections between school staff, who know their students and families' best, and mental health service providers committed to serving their local communities. This will allow Woodhull Child and adolescent clinic to see more students in need of urgent evaluation, assessment, treatment and referrals for additional support.

AREAS OF RESPONSIBILITIES:

Work together to provide comprehensive, personalized care for all New Yorkers and commit to providing a person-centered care environment while exemplifying ICARE values
• Conducts assessments with patients and/or family members(s)/significant others to determine care needs
• Provides individual, family, and possibly group psychotherapy, including crisis and grief counseling to school-aged children in the Brooklyn
• Advocates and serves as a liaison for patients with community agencies.

SUMMARY OF DUTIES AND RESPONSIBILITIES:
• The duties include functioning as a primary therapist for subset of patients treated in the Outpatient Clinic, facilitates group therapy, conducting initial psychiatric assessments to determine care needs, documenting biopsychosocial assessments, formulation and disposition, developing comprehensive treatment plans, writing discharge summaries and other related paperwork. This also includes mandated reporting.
• Provides clinical supervision of the assigned social work II staff of the mental health continuum
• Ensures adequate coverage and continuity of patient care at all times.
• Participates in the formulation of standards, polices, and procedures for orientation, supervision and in-service training of social work staff.
• Provides orientation to new staff and updates incumbent staff of changes in policy, procedure, or standards.
• Knowledgeable of departmental, facility, network, city, state, federal and JCAHO standards and regulations.
• Monitors staff activities to ensure compliance with all the above.
• Provides information about and acts as linkage to available community resources.
• Ensure staff documents all interventions and continuing care plans in the patient's medical record in accordance with departmental and hospital policies.
• Completes performance evaluations for assigned staff.
• Maintains and completes statistics and reports.
• Maintains a caseload as required
• Conducts and/or attends individual, unit, departmental and intra/inter hospital meetings as mandated by the department's administrator.
• Participates in the department's cross-training program.
• In collaboration with select schools, the Social Worker III will provide timely mental health clinic appointments, urgent care, and tele-video visits to the students while in school or at home.
• The Social Worker III will participate in clinical supervision with Social Worker Level IV/Associate Director.
