Job Description

TOUR (Rotating)

PURPOSE OF POSITION:

Provides Palliative Care/Social Work services and the coordination of continuing care planning, utilizing a variety of techniques such as individual, family, group work, and community networking to patients serviced at this facility.

AREAS OF RESPONSIBILITIES:

Palliative Care team consult and outpatient services

SUMMARY OF DUTIES AND RESPONSIBILITIES:
• Review charts of patients referred or consulted for palliative care services.
• As an active member of the Palliative Care team, the SW will participate in Palliative Care Interdisciplinary rounds with MD, Pastoral Care, etc.
• Interview and evaluate a patient and/or family/HCP/surrogate psychosocial needs, stressors, etc.
• Assist the Palliative Care team in facilitating patient-family meetings.
• Assists attending physicians, medical team, SW staff, patients and family in the clarification of the goals and the plan of care.
• Provides anticipatory bereavement/ bereavement counseling (individuals and/or group) to patients and their families.
• Provides referrals to other counseling and education in hospice and grief resources in the community.
• Provides information on burial resources, death benefits and other logistical information as needed.
• Provides education and counseling, regarding palliative care and bereavement issues for patients, families as needed.
• Provides related concrete services as well as supportive and crisis intervention/counseling, to patients and families for end-of-life care and/or safe discharge planning.
• Confers with medical, nursing, and other professionals, in interdisciplinary rounds, to develop an assessment and care plan towards a safe discharge.
• Provides consultation and education for healthcare professionals on Palliative Care service
• Act as a liaison for the hospital with hospice care representatives to keep abreast and up to date on community hospice resources.
• Communicates with community agencies about provisions of services. Assist to initiate referrals on behalf of patient in collaboration with unit/clinic Social Worker.
• Ensure documentation of interventions provided via EPIC
• Keeps statistics and prepares reports of Palliative Care activity and productivity
• Attends and presents in Palliative Care conference rounds, departmental, other interdisciplinary meetings, and in-service education as needed
• Participates in Ethics Committee meetings as assigned.
• Provides supervision to Social Work interns/students and Social Worker level II as assigned
• Participates in performance improvement activities and monthly meetings.
• Attends individual, unit, departmental and intra/inter hospital meetings as mandated by the supervisor and/or department's director. Represents the department in meetings as requested.
• Assists/Participates in presentations, training, committees, special studies, research and in-services as required
• Complies with requirements for supervision, training, Performance Improvements Projects, Quality Assurance and Research Studies.
• Participates in the department's cross-training cross-coverage program.
• Assist in the orientation and training of new staff of the SW department and other disciplines as needed.
• Understands and follows the escalation process as needed.
• Keeps abreast and participates in performance improvements initiatives within the network.
• Perform other duties assigned by the social work administration.

Minimum Qualifications

For Assignment to Level III
1. Valid license as an LCSW issued by the NYSED.
