Job Description

Provide biopsychosocial assessment per referral and assessment policy, plan and carry
out appropriate interventions.
• Complete required documentation in a timely fashion, according to assigned area
requirements and in a professional manner (see documentation policy)
• Provide ongoing support and psychotherapeutic counseling to patients,
families/significant others as well as concrete services at frequency determined by
respective assigned area.
• As indicated, make contact with patient's families, collaterals, significant others, as well
as community and medical providers and include/share this information in assessment,
documentation, and during multidisciplinary meetings.
• Collaborate with family, collaterals, and providers in terms of treatment, referrals and/or
aftercare planning.
• Escort patients into the community and make home visits as needed.
• Attend rounds and meetings with multidisciplinary teams.
• Advocate and negotiate with related social and community agencies to ensure services for
patients.
• Provide caring, nonjudgmental, and ethical services to patients.
• Participate in performance improvement projects
• Comply with all department, corporate, and hospital regulations, policies and procedures
and conforming to established dress guidelines, time and leave requirements as an
essential worker, and wearing identification badge while on duty.
• Incorporate universal screening for patients to identify if they are a victim of abuse.
Follow hospital policies on interventions regarding child abuse, intimate partner violence,
elder abuse, sexual assault, and human trafficking as well as NYS protocols for mandated
reporting of suspected child abuse or maltreatment.
• Participate in regular supervision and ad-hoc supervision/escalation as necessary to seek
guidance from appropriate personnel.
• Perform work-related duties with the understanding that as a hospital employee, one is
categorized as an essential worker, which includes coming to assignment under a variety
of circumstances such as inclement weather, pandemic, and other extreme circumstances.
• Other tasks as assigned.

Minimum Qualifications

For Assignment to Level II
1. Valid license as an LMSW issued by the NYSED; or
2. Valid license as an LCSW issued by the NYSED.
