CLIENT'S SUMMARY OF THIS OPPORTUNITY

• Provides direct/indirect services to recipients such as: intake assessment and ongoing evaluations that include obtaining personal, family, social, psychiatric, substance abuse and medical information.
• Conducts Targeted Case Management and Supports Coordination functions to ensure continuity of care and services for each recipient. These functions include assessment, service plan development, assisting the recipient in gaining access to needed medical, social, educational, financial and other services, linking and coordination of these services.
• Assures that all recommendations and services identified in the assessments are addressed in the Individual Plan of Service.
