Job Description
Purpose of Position:
Under supervision, may supervise social work staff as assigned. Provides social work services and the coordination of continuing care planning, utilizing a variety of techniques such as individual, family, group work, and community networking to patients serviced at this facility.
Areas of Responsibilities:
Medical/Surgical/MCH/Peds/Inpatient/Outpatient/SUD
Summary of Duties and Responsibilities:
Accept referrals and case finds potential SUD cases.
• Reviews the medical records of patients in order to identify those who will require SUD intervention and counseling
• Participates in daily Interdisciplinary Rounds at inpatient units to provide team with insight into patient's psychosocial functioning and obtain understanding of patient's SUD needs and insights.
• Interviews patient to assess their insight into their addiction and readiness to accept treatment.
• Initiates pt.'s referrals to appropriate in house or community-based SUD programs.
• Completes Social Work Assessment for these high/moderate-risk patients as needed.
• Maintains effective communication and provides supportive counseling, crisis intervention to the patient with regards to addiction treatment options and services
• Collaborates with unit social worker on social work treatment/discharge plan which may involve the patient, family/significant other and the interdisciplinary team.
• Collaborates as a liaison between in patient med/surg and Chemical Dependency department staff/programs
• Documents all Social Work interventions in the patient's electronic medical record in accordance with departmental and hospital policies.
• Confer and consults with unit SW and clinical team in the development of services for the patient.
• Maintains effective communication with interdisciplinary team and participates actively of interdisciplinary discharge planning rounds and records plans/recommendations in the patient's medical record.
• Coordinates individual interdisciplinary case conference as needed.
• Provides supervision to Caseworkers, social work students and Social Workers Level I when designated. Acts as a resource for SW staff.
• Provides continuity of care and aftercare services to patients beyond admission as needed.
• Will provide individual and group therapy to patients receiving services through Chemical dependency department as needed.
• Assist with chart reviews and reports as requested.
• Maintains and completes statistics of case activities as outlined in the departmental policy
• Attends individual, unit, departmental and intra/inter hospital meetings as mandated by the supervisor and/or department's director. Represents the department in meetings as requested.
• Assists/Participates in presentations, training, committees, special studies, research and in-services as required
• Complies with requirements for supervision, training, Performance Improvements Projects, Quality Assurance and Research Studies.
• Participates in the department's cross-training program.
• Assist in the orientation and training of new staff of the SW department and other disciplines as needed.
• Understands and follows the escalation process as needed.
Minimum Qualifications
For Assignment to Level II
1. Valid license as an LMSW issued by the NYSED; or
2. Valid license as an LCSW issued by the NYSED.
